    Scenario
    A large fast-food chain unveiled a new touch screen register for its franchises. Each cashier was assigned a user ID and password combination to log in to the register. The system allowed the incorrect password to be entered four times before the register would lock and require a manager to unlock it with a key card. To prevent unauthorized access, the registers would lock after three minutes if the screen was not touched. When the register locked, only the cashier who was logged in prior to it locking could unlock it without a system restart.
    After a few days in operation, restaurant managers started complaining about the amount of time they were spending unlocking the registers. Some cashiers were forgetting their user ID and password, so other cashiers would log in for them. It also seemed that the button layout made it easy for the cashiers to key in the incorrect password. The managers also complained that cashiers would leave for a break or end their shift and forget to log out of the locked register. The managers would have to reboot the system, a three- to five-minute process in order for the next cashier to log in. Additionally, managers noticed that grease was building up on the touch screens, making them less responsive.

You’ve been asked to put together a team to design optimal work facilities for the department’s newly leased space in a neighboring building. You have four months to put your plan together before the lease begins. You may choose four other people from the company to be on your team. For this assignment, create the names of the people you choose. Assume that you know each person and can explain why you chose them. You can base your descriptions on real people that you know if you wish.

Part 1: Write a short paragraph briefly describing why you chose the four members of your team. Did you choose based on MBTI, past work history, emotional intelligence, office politics, ability to work on a team, or some other reason? There are no wrong answers. Summary: A theory can be used to guide practice.  This assignment is an exercise in supporting a clinical practice with theory and evidence.

Directions: Identify an outcome of nursing practice in your area of practice that can be improved.  For example, if you work in home health, you may identify that throw rug use by fall risk patients is too prevalent.  You may be able to use the problem that inspired the theory concepts that you developed in week two.

Briefly support why it is a problem with evidence from the literature. This is not the major focus of the assignment so do not elaborate.

Create a clinical nursing (not medical) theory in the form Concept A | Proposition | Concept B. Think of the structure like two nouns and a verb. While the term proposition is much more complex in the dictionary, in our use it is the connecting term between the two concepts. Examples include Concept A improves Concept B, Concept A is related to Concept B, when Concept A increases then Concept B also increases, etc. When you get to research, you will explore this further as you develop independent and dependent variables. How to use these statistically will come in research and statistics courses.

This clinical theory is identified as an empirical theory when you get to the C-T-E model later in this course. It is empirical in that they can be measured.

Identify and define your concepts. Identify how they could be measured in a research study. Be careful that you do not use compound concepts. If you find the words “and” or “or” in your theory, you are probably too complex.

If you research your question and seek funding, you will need a theoretical model to guide the research. In our assignment, we are using Watson. You will identify the concepts in Watson’s theories that are represented by the concepts you are using in your clinical theory. Match the proposition in her theory with your proposition. To help, the 10 Caritas Processes are Concept A. Choose the one that matches your concept. To clarify, let’s look at Caritas 1 Embrace and use it in middle-range theory. Sustaining humanistic-altruistic values by the practice of loving-kindness, compassion, and equanimity with self/others (Concept A – Very complex and abstract) improves (Proposition) subjective inner healing (Concept B). 

Remember that the paper is not about the problem. It is about constructing a clinical theory and matching it to a middle-range theory and conceptual model your clinical theory represents.

Conclude the paper with your discoveries made in your readings and the impact on the nursing profession of your discoveries. Explore, briefly, discovered questions that require further research. Summarize the paper in the conclusion.

Present your outcome in an APA formatted paper that meets the University’s standards for a written assignment.

  • Length: 7 pages including title and reference pages
  • References: 3 to 10.  There should be enough to support the links between the concepts of the problem and the concepts of Watson’s Theory of Caring.

 Mrs. Wong, a first-time mother, is admitted to the birthing suite in early labor after spontaneous rupture of membranes at home. She is at 38 weeks of gestation with a history of abnormal alpha-fetoprotein levels at 16 weeks of pregnancy.  

She was scheduled for ultrasonography to visualize the fetus to rule out an open spinal defect or Down syndrome, but never followed through. Mrs. Wong and her husband disagreed about what to do (keep or terminate the pregnancy) if the ultrasonography indicated a spinal problem, so they felt they did not want this information. 

Reflective Questions

 1. As the nurse, what priority data would you collect from this couple to help define relevant interventions to meet their needs?

 2. How can you help this couple if they experience a negative outcome in the birthing suite? What are your personal views on terminating or continuing a pregnancy with a risk of a potential anomaly? What factors may influence your views?

Evidence based intervention method. 

Identifying a population and the intervention need is the first step in evidence based intervention. The selected population in this case is African Americans living in Hialeah, Florida. Life expectancy of the selected population has drastically reduced due to heart diseases and complications in the past ten years (Chamberlain College of Nursing, 2020). Heart diseases is a major concern in the African Americans especially in this region. It is the leading cause of mortality in Hialeah (Mendy, Vargas & El-sadek, 2016). A lot of health disparities are faced by the selected population. This is because the population lacks proper formal education which results in illiteracy in matters concerning their health. In addition, the lack of formal education means that majority of the population cannot get formal employment due to lack of skills further leading to poverty. 

From this assessment, it is clear that the selected population needs an intervention to reduce disparities in health care. There exist differences among races in terms of health care. However, from research it is clear that most adults in the selected area have relatively poor cardiac health (Short, Gamble & Mendy, 2013). The aim of Healthy People 2020 is to improve cardiovascular health by treatment, detection and prevention of associated risk factors (Short et al., 2013). Prevention can be done by smoking cessation, maintaining a healthy weight, physical exercises and eating healthy (Short et al., 2013). Detecting the disease can be done by regular checkups according to the schedule laid out by the physician. Hypertension, hypercholesterolemia, diabetes and hypertension are the associated risks of the disease that can be treated. Educating the population on treatment, detection and prevention of the associated risk factors of the heart disease can aid in implementing a prevention plan. 

Ways in which the intervention method addresses the CLAS standards. 

The intervention plan I have selected seeks to detect, prevent and treat risk factors linked to the cardiac disease. All interventions included address the standards of CLAS by continuous improvement, accountability and promoting engagement. Treatments, detection and prevention strategies encourage patient centered care and patient involvement. In order to achieve the intended goal of the intervention the selected population will have to be educated on the heart disease. It is important for the people to understand risk factors of the disease in order to be better prepared. Medium language of communication will be simple English. Further explanation of medical terminologies will be used in case the selected population is having difficulties understanding them. Goal here is to use the simplest language to make the population better understand the medical issue at hand. The Intervention plan will also seek to address standards of communication used in CLAS. 

Professional experience on the topic. 

According to my experience heart disease is a prevalent issue in many African American communities. It is attributed to poor health literacy. For instance, majority of the population is not aware of the different ways of preventing detecting and treating the heart disease and other associative factors. I view it as part of my duty as a professional to educate families and individuals on how to prevent the disease. Reason being I also have members of my family too affected by the cardiac disease. So the issue is of importance to me. By educating others I get to help in stabilizing and maintaining the disease. As a professional I have also observed that humanity at large is a creature of habit. A patient may be educated and have a deeper understanding of the potential risks associated with the disease but will not pay attention. It is not until a scary event happens that an individual will want to change their behavior and lifestyle. For example, it may take a patient to suffer from a heart attack for them to completely quit smoking. Therefore, it is crucial for patients to be reminded time and again the risk factors associated with the disease. Further, patients who have gone through a near fatal experience can share their experiences in order to reinforce the idea. It is a good way for individuals to learn of what they are putting at risk.
