Content Review

Directions:

  • Respond to each item. Each response should be concise and between 2–3 paragraphs in length.
  • Use MS Word to write your responses, and submit your answers to all three questions in one Word document.
  • Copy and paste each question within the document, so that your Instructor can see which question you are responding to.
  1. In Chapter 1 of Diversity in Early Care and Education, Janet Gonzalez-Mena introduces the concept of transformative education and discusses the role of dialogue. Review the information presented on pages 25–26 (including Point to Ponder 1.9). Using an example related to the care of infants or toddlers that you have read about or observed in the first four weeks of this course, explain what the terms transformative education and dialogue mean, as well as how these concepts relate to each other.
  2. Differences between individuals’ concepts of time, which are shaped by culture, can lead to conflicts. Review the information on time concepts presented on pages 38–39 of Diversity in Early Care and Education and then review the information on outcomes to cultural conflicts addressed on pages 54–56 of the text. Describe one or more conflicts that may arise between an infant/toddler professional and a family member if the two individuals have different views on time. Then, using this example, compare and contrast two or more of the five outcomes described on pages 54–56, and explain the advantage(s) of one outcome over the other.
  3. On page 56 of Diversity in Early Care and Education, Janet Gonzalez-Mena refers to the challenge issued by the National Association for Education of Young Children (NAEYC) for infant/toddler professionals to move beyond dualistic thinking toward holistic thinking. Using an example related to the care of infants or toddlers, explain what holistic thinking means and describe how it presents a better alternative for addressing the needs of everyone involved.

Reflective Questions

Instructions: Read the following case study and answer the reflective questions. Please provide rationales for your answers. Make sure to provide citations/references for your answers in APA format.

CASE STUDY: Active Labor: Susan Wong

Mrs. Wong, a first-time mother, is admitted to the birthing suite in early labor after spontaneous rupture of membranes at home. She is at 38 weeks of gestation with a history of abnormal alpha-fetoprotein levels at 16 weeks of pregnancy. She was scheduled for ultrasonography to visualize the fetus to rule out an open spinal defect or Down syndrome, but never followed through. Mrs. Wong and her husband disagreed about what to do (keep or terminate the pregnancy) if the ultrasonography indicated a spinal problem, so they felt they did not want this information.

 Reflective Questions

1. As the nurse, what priority data would you collect from this couple to help define relevant interventions to meet their needs?

 2. How can you help this couple if they experience a negative outcome in the birthing suite? What are your personal views on terminating or continuing a pregnancy with a risk of a potential anomaly? What factors may influence your views?

3. With the influence of the recent Human Genome Project and the possibility of predicting open spinal defects earlier in pregnancy, how will maternity care change in the future?

Application of Systems Theory to a Case Study

In this course, you will be asked to select one case study and to use it throughout the entire course. By doing this, you will have the opportunity to see how theories guide your view of a client and the client’s presenting problem. Although the case may be the same, each time you use a different theory, your perspective of the problem changes, which then changes how you go about asking the assessment questions and how you intervene.

The first theoretical approach you will use to apply to a case study is systems theory. In other words, your theoretical orientation—your lens—will be systems theory as you analyze a social work case study.

Different theories can be used to take a systems approach. For example, Bertalanffy’s General Systems Theory considers how a system is made of smaller subsystems that influence each other and seek homeostasis, whereas Brofennerbrenner’s Ecological Systems Theory focuses on how an individual’s experience is influenced by different system levels (micro, meso, exo, macro, and chrono). Systems theory is commonly used to understand the interrelationships of the systems (e.g., family, community, organizations, society) of the client. If you are working with families, communities, and organizations, it is also beneficial to use systems theory to get a holistic picture of all the interrelated parts of the system.

To prepare: Select and focus on one of four case studies listed in the Learning Resources. You will use this same case study throughout the course.

  • Focus on the identified client within your chosen case ( Tiffani Bradley  )
  • Analyze the case using a systems approach, taking into consideration both family and community systems.
  • Complete and submit the “Dissecting a Theory and Its Application to a Case Study” worksheet based on your analysis.

Assignment 11: Reverse Referral Fees

HA4070D – Regulatory Environment in Health Care

Assignment 11: Reverse Referral Fees

Assignment

Task: Submit to complete this assignment

Directions

Complete the Problem on page 573 “Reverse Referral Fees” as a two to three-page paper. A general description of the Marcus Welby Hospital is located here.

You are outside counsel to the Marcus Welby Healthcare Corporation (MWHC), which is concerned that expenses in some of its ancillary departments are causing it to lose money under Medicare and HMO insurance. It would like to start charging its hospital-based physicians for some of the costs of running their departments. Its current relationship with these physicians is one in which they have exclusive contracts to work in these departments, but no money changes hands between them. The hospital handles all billing, staffing, and overhead, but it bills separately for facility charges versus professional fees, and the physicians keep all the professional fees the hospital collects on their behalf. This is the standard practice in the industry. MWHC has the following suggestions for changing this arrangement:

• Have the radiology group pay for services, supplies, personnel, utilities, maintenance, and billing services furnished by the hospital. In a non-hospital, office-based setting, this package would normally cost about $100,000 to $150,000 per year. The hospital will charge the radiology group only $25,000 at first, but increase the charges to $100,000 over four years. Payments are due only if the hospital’s gross revenue derived from radiology services exceeds $1,000,000 in the previous year.

• The hospital’s clinical laboratory, under the direction of the pathology group, would pay the hospital a 20 percent fee for “specimen collection and handling services” when a physician on the MWHC medical staff orders a test from the clinical lab.

Insomnia is one of the most common medical conditions you will encounter as a PNP. Insomnia is a common symptom of many mental illnesses, including anxiety, depression, schizophrenia, and ADHD (Abbott, 2016). Various studies have demonstrated the bidirectional relationship between insomnia and mental illness. In fact, about 50% of adults with insomnia have a mental health problem, while up to 90% of adults with depression experience sleep problems (Abbott, 2016). Due to the interconnected psychopathology, it is important that you, as the PNP, understand the importance of the effects some psychopharmacologic treatments may have on a patient’s mental health illness and their sleep patterns. Therefore, it is important that you understand and reflect on the evidence-based research in developing treatment plans to recommend proper sleep practices to your patients as well as recommend appropriate psychopharmacologic treatments for optimal health and well-being.

For this Discussion, review the case Learning Resources and the case study excerpt presented. Reflect on the case study excerpt and consider the therapy approaches you might take to assess, diagnose, and treat the patient’s health needs.

Case: An elderly widow who just lost her spouse.

Subjective: A patient presents to your primary care office today with chief complaint of insomnia. Patient is 75 YO with PMH of DM, HTN, and MDD. Her husband of 41 years passed away 10 months ago. Since then, she states her depression has gotten worse as well as her sleep habits. The patient has no previous history of depression prior to her husband’s death. She is awake, alert, and oriented x3. Patient normally sees PCP once or twice a year. Patient denies any suicidal ideations. Patient arrived at the office today by private vehicle. Patient currently takes the following medications:

  • Metformin 500mg BID
  • Januvia 100mg daily
  • Losartan 100mg daily
  • HCTZ 25mg daily
  • Sertraline 100mg daily

Current weight: 88 kg

Current height: 64 inches

Temp: 98.6 degrees F

BP: 132/86

Post a response to each of the following:

  • List three questions you might ask the patient if she were in your office. Provide a rationale for why you might ask these questions.
  • Identify people in the patient’s life you would need to speak to or get feedback from to further assess the patient’s situation. Include specific questions you might ask these people and why.
  • Explain what, if any, physical exams, and diagnostic tests would be appropriate for the patient and how the results would be used.
  • List a differential diagnosis for the patient. Identify the one that you think is most likely and explain why.
  • List two pharmacologic agents and their dosing that would be appropriate for the patient’s antidepressant therapy based on pharmacokinetics and pharmacodynamics. From a mechanism of action perspective, provide a rationale for why you might choose one agent over the other.
  • For the drug therapy you select, identify any contraindications to use or alterations in dosing that may need to be considered based on ethical prescribing or decision-making. Discuss why the contraindication/alteration you identify exists. That is, what would be problematic with the use of this drug in individuals based on ethical prescribing guidelines or decision-making?
  • Include any “check points” (i.e., follow-up data at Week 4, 8, 12, etc.), and indicate any therapeutic changes that you might make based on possible outcomes that may happen given your treatment options chosen.

Discussion – Positive Attitude

Discussion: Unit 3,

Positive Attitude

The role models you identify as “successes” did not get there without adversity, obstacles, and challenges. They simply focused on their outcome and did not allow themselves to become sidetracked with guilt or excuses.  “Ask and you shall receive, seek and you will find.” They focused on exactly what they wanted and formulated the internal belief and attitude that it was attainable. The common denominator to success and life balance is a positive attitude.

Did you know that it takes the same amount of energy to formulate a positive thought as it does a negative one?  Positive thinking is ultimately about structuring your belief system; that is, substituting positive images for negative ones.  It is the process of consciously filtering out unhealthy thoughts and cleansing the mind.

For the discussion, find a quote about attitude on the website (see link) that resonates well with your personal views on what you believe about your current attitude (or what you want your attitude to be).

Then, answer these questions about your quote in 3-5 sentences:

  1. Explain to your classmates why your quote is important to you.
  2. How will your quote motivate you when you face a challenge to accomplishing your academic goals?
  3. How will your quote change your attitude?
