Review Healthy People 2020 Global Health Initiatives. Choose one disease process and discuss the disease process, the goal of Healthy People 2020, and how the CDC plays a role in meeting the goal.

The disease process I chose to discuss is heart disease and stroke. Heart disease is caused by various risk factors including, smoking, hypertension, high cholestrerol, poor diet, and inactivity. Patients can also be genetically predisposed to heart disease. Healthy people 2020 has set a goal and interventions to help reduce the amount of people affected by this disease and to improve the quality of life of those that are affected. Improve cardiovascular health and quality of life through prevention, detection, and treatment of risk factors for heart attack and stroke; early identification and treatment of heart attacks and strokes; prevention of repeat cardiovascular events; and reduction in deaths from cardiovascular disease (Healthy People 2020, 2018). This goal can be met by patients becoming aware of their risk for heart disease. This takes place by having screenings such as labs to check for high cholesterol, smoking cessation counseling, and management of high blood pressure. The CDC supports Healthy People 2020 in reaching their goal. With $165 million for heart disease and stroke programs (including WISEWOMAN) in Fiscal Year 2018, CDC supports all 50 states and the District of Columbia to conduct heart disease and stroke prevention efforts (CDC, 2018). The goal can be met with interventions being implemented not only by patients but healthcare providers. Healthcare providers must use screenings to determine the risk of heart disease. Also important would be educating the patient of the effects of the disease and management of the disease including compliance with medication such as a daily regimen of aspirin if not contraindicated. Heart disease and stroke are big topics in the United states that need awareness and initiatives for meeting goals. In addition to being the first and fifth leading causes of death, heart disease and stroke result in serious illness and disability, decreased quality of life, and hundreds of billions of dollars in economic loss every year (Healthy People 2020, 2018).

Research Design in Social Work

 Early adulthood goes up to about 35 years of age, so your study should not include people over the age of 35. Your population is probably infinite, you must investigate the census to see the estimated population. If you do not succeed then we can assume that it is infinite in such a way that your total sample must be 384 participants if you decide to have a reliability level of 95%. This is something you should consider. The reliability level will determine your sample. We discussed it in the last class.

You have to work a little harder on your whole methodology. You have to be specific. How are you going to collect the data, what are the steps, how are you going to analyze it?

Discussion: Prejudice

People often use jokes to test the boundaries of what is considered offensive. Under the guise of humor, the joke teller can attempt to minimize any offense taken by defending the humor as “just a joke.”

Clearly, an attempt to belittle or demean others as a form of humor is distasteful. And in every culture, one can encounter humor that, to be understood, relies on prejudice—whether that prejudice be about race, sexual orientation, gender, ethnicity, or nationality. What others might find funny, some find tasteless and indefensible. Simply because an offensive statement is clothed as a joke does not make the joke funny nor does it mean that the joke teller cannot be confronted by someone who finds the comment offensive or inappropriate. Even so, confronting someone about an inappropriate joke can present a social risk; the person being called out is likely to feel defensive and retaliate against whomever exposed them.

For this Discussion, you will draw on your own experience and social psychology theory to examine responses to prejudiced comments.

 Discuss the level of oversight on higher education institutions imposed by federal and state government and regional accrediting agencies. Do you feel this level of oversight is justified? Why? Do you feel like there should be more oversight or less, and who should be responsible for insuring colleges and universities meet up to the goals they have established? Who should set the goals for higher education institutions? Should higher education institutions be assessed more like many public K-12 institutions?

The level of oversight on higher education institutions imposed by federal and state government focus more on the thresholds and allowances for Title IV funding and affordability.  With the far reaching improprieties of for profit schools culminating in campus closures with little to no notice to the student, it seems as if the federal government is doing little to regulate tuition costs and work force development, and leaving that up for the states to regulate.  As a College Director, I see much more stringent governance from the State of Texas Workforce Commission with its routine campus visits to review refund calculations and faculty credentialing, compliance, and other organizational concerns than I do from a federal agency.  Career colleges much like any other higher educational institution are regionally accredited and in most cases programmatically accredited.  These agencies have greater concerns with outcomes such as completions rate, graduation rate, employment/placement rates etc. This oversight helps to keep organizations focused on the quality of education and ensuring that graduates are gainfully employed and able to manage their college debts when they come out of college.  The Department of VA-Educational Services division provides an additional prong of oversight to ensure veteran students are treated fairly and are afforded a quality educational experience.  This agency alone can be a nightmare to deal with when you are facing a grievance from a student(s) receiving Post-911 GI benefits.  I don’t feel there is a shortage of oversight effecting the higher education institutions; but a huge deficit in those who are tasked with governance who truly understand the needs of the student learner experience and sincere effort to meet the desired outcomes of gainful employment, affordable education, skillful labor force meeting the industry demands of today and the future.  If anything we should be seeing more oversight towards financial and fiduciary responsibilities of board members and shareholders. The recent debacle with ECA-Education Corporation America, announcing a teach out of over 50 campuses, right on the heels of my own employer-Zenith Education Group announcing the closure of all but three of its 23 schools certainly makes you question the business acumen of those residing at the top levels of the institutions.

Goals for the higher education institutions should be set by the same accrediting agencies, state and federal constituents as they have previously been with a critical look at the individuals who are in the positions of managing the process of goal setting. We must stop putting people in positions of authority and governance who do not care about education reform, affordability, accountability and outcomes. If you are unable to articulate and demonstrate a knowledgeable, successful track record in these areas, then the agencies regulating and providing governance will continue to fail.  Our regulatory bodies for higher education are filled with people who just don’t have the moral turpitude or experiential background to be where they are and most importantly; they have no desire to ensure that our educational system is being run properly.

Personhood

The concept of personhood, as defined by Schoenhofer, is a central concept of the theory of Nursing as Caring Theory.  Nursing as Caring Theory is grand nursing theory developed by Theorists Boykin and Schoenhofer that evolved from theories of Paterson and Zderad (Duffy, Donnell, & Snowden, 2014).  The ideologies of these theorists are cultivated around a care model that evaluates and defines nursing as a discipline and a profession as it relates to the patient and the nurse.

The field of nursing has progressed from a bedside profession to an advance art of discipline. The evolution of nursing from a profession to an advance discipline was made possible due to advancement, analysis, and applications of principles of medicine and philosophy. Philosophically personhood is defined as the status of being a person.  Nursing embodies this concept and adapts it to each individual meaningfully and appropriately.  Theorists Boykin and Schoenhofer developed the grand nursing theory of Nursing as Caring around the central ideas of “the between”, “the call for nursing”, ”the nursing response”, and “personhood” (Duffy, Donnell, & Snowden, 2014). These four central ideas are interconnected and offer a conceptual framework in caring for patients and enhance and afford the ability for all those involved in the nursing process the ability to be “a person”; an integral role for the nurse and patient because without this authenticity the nursing process will not be able to reach its fullest potential (Schoenherr, 2002).

The call of nursing is the initial step in the methodology of the Nursing is Caring Theory.  It is during this stage the nurse accepts the ‘call’ to nurture.  The call of the nurse is then proceeded by the ‘nurse’s response’. This nurse response is a testament to all her knowledge and experiences and the ability to translate and relate to the task at hand.  Once the nurse’s knowledge and experiences are uniquely appropriated for the environment, trust is gained from the patient and the acknowledge they are seen as a person and able to exhibit their personhood to the nurse.  Nurses must do this on a daily basis and multiple times within in the day due to the various dynamics they are faced with, different patient demographics, their caregivers, and other staff members.  When a nurse enters the environment of the patient it is where they are most vulnerable and allowed to exihibit personhood and adapt their abilities and develop their abilities to care for the patient.  It is in this raw instance of vulnerability and personhood that a nurse is able to be receptive and learn the needs of the patient and afford them the best care.

The most impactful and meaningful takeaway from Nursing as Caring Theory is that, above all else, nurses instinctively and inertly will act upon the intent to help, heal, and nurture.  Due to the ubiquitous nature of the discipline and profession of nursing, a nurse can easily adapt their nursing response to be suitable for the environment and call of nursing and appropriate to be purposeful for each individual.  This flexibility and adaptability of a nurse is a skillset that facilitates the holistic art of nursing that is conducive to learning, teaching, healing, and promoting wellness in individuals.

The case study of George, the social worker meets the criteria for a single-case design, because the focus is on a single person (Plummer, Makris, & Brocksen, 2014).  The study objective was to test the efficacy of one intervention with multiple target problems (Plummer, Makris, & Brocksen, 2014).  The research design is a multiple baseline design (MB), which means that the intervention presented to George in the case study was applied to each sequential target problem and allowed time to assess the impact of the intervention on each issue before introducing it to the next challenge. There were no specific tasks given, but it is stated that there were two personal care tasks completed, two mobility tasks and three home safety tasks. Progression was monitored on each visit using a 10-point assessment scale (Plummer, Makris, & Brocksen, 2014). I would support the use of the clinical rating scale in the case study, as it was shown that this type of measurement scale is one that is commonly used. In the design of a single system and was effective and reliable as well as desirable by several other researchers dealing with unique device designs (Mattaini, 2010). The Goal Attainment Scale (GAS) can be an effective form of assessment, as it would measure multiple targets for a particular adult such as George, considering his mobility, health, and personal care (Mattaini, 2012; Plummer, Makris, & Brocksen, 2014). Another valuable measurement tool would be a Rapid Assessment Instrument (RAI), which would allow data to be gathered rapidly and routinely (Mattaini, 2012). This enables the monitoring and recognition of incidents within a case through its functional relationships (Mattaini, 2012).

The case study did not provide details about the methods they used. Without these specific details, it is unclear whether the operation and the research were ethically undertaken (Plummer, Makris, & Brocksen, 2014).  I did notice substantial changes in George’s home safety conduct when assessing the social worker’s use of a strategy composed of case management and solution-focused and task-centered methods. I have observed significant improvements in his behavior in personal care and a slight increase in his mobility. These results suggest that the strategy chosen has been successful in maintaining home health, effective in strengthening personal care and helping to improve mobility. Given the importance of addressing these issues in my neighborhood and the lack of funding at this moment, I will advocate a tailored case management intervention program for home health, personal care, and mobility. Chris and her collaborators will need to prove that the research and the findings can be repeated and applied to other populations to be considered as evidence based and therefore, to be considered for submission to the registry (Tankersley, Cook, & Cook, 2008).  To do this, I recommend identifying the measures used in the treatments and how they were performed based on length, and location. Through providing detailed data, other researchers are more able to replicate the sample to see if it can be used with different populations to show generalizability and to see if outcomes are like the original analysis for replication.
