Scenario

Every leader uses their traits, behaviors, and the circumstances of the situation to achieve successful outcomes. Organizational change is desirable to remain competitive, and leaders serve as change agents to ensure these changes are successful. You have been recently selected to lead an organization’s change efforts concerning the recent pandemic. During the pandemic, the organization moved to a work from home (WFH) model.

It has been several months now, and the WFH model proves to be highly successful for the company. Now the pandemic is gone, and the move back to the office is possible without sacrificing employee health and safety. Executive leadership has decided to continue the WFH module for the indefinite future; however, some managers in the organization want to move back to the office versus the continued working from home model.

The CEO has tasked you to promote this change for the organization and encourage other managers to get on board. You are serving as the change agent leader for this project by creating a presentation for an upcoming manager kickoff meeting for the work from the home model initiative. Remember, the goal is to create “buy-in” and support from managers on this new initiative.

The Affordable Care Act and the American Health Care Act Compared

The provisions of the Affordable Care Act (ACA) directs all employers to provide minimum value and affordable medical cover to all full-time employees or else be penalized whereas the American Health Care Act (AHCA) removed the penalty on defaulting employers (Conley, Dygert & Sellstrom, 2016). Under the ACA, it is the people’s mandate to buy healthcare coverage whereas the AHCA abolished the penalty requiring individuals to purchase health cover. However, the AHCA issues penalties to individuals who fail to pay their healthcare coverage for more than 63 days in the last 12 months, and youth who are past the age limit under parental cover and did not enroll for health cover of that year (Conley, Dygert & Sellstrom, 2016).

The penalty is a 30% surcharge in premiums, which can last for a period of 12 months (Conley, Dygert & Sellstrom, 2016). On the aspect of coverage affordability, ACA provides for cost-sharing subsidies in low-income families or individuals to allow for exchange plans for up to an average of 70%. The premiums to be paid by such individuals and families have tax reductions to enable low-income earners afford healthcare insurance (Mach, 2017). On the other hand, the AHCA offers credits on the basis of age with fixed amounts such as below 30 years-$2000, 30-40 years-$2500, $3,500-50-60 years, and it faced out credits for people earning incomes that exceed $75,000 (Conley, Dygert & Sellstrom, 2016). The ACA removed additional charges for health status whereas AHCA charges 30% standard rate for people with preexisting health complication for the past 12 months (Mach, 2017).

Opinion on Why AHCA Failed in the first Passage Attempt

           The most likely reasons for the failure of AHCA during its first passage attempt were due to its provisions that would impact negatively in low income families and the less-privileged individuals with regard to their preexisting health status. For instance, the AHCA lifted penalties that employers were to be charged if they defaulted paying the compulsory health cover for all their permanent employees. This implies that employees could pay their employees’ cover only when they wished, which could push the burden back to employees. The other reasons are that people with preexisting medical conditions would be required to pay more premiums, and abolished sharing of insurance cover, and charging premiums based on age instead of economic capabilities. This means that you can be charged little premiums even if you earn high, and you can be charged high premiums even if you poor depending on your age.

Through his studies he has explained that every individual has his or her own unique condition for good sex. You must look at both conditions sexual satisfaction of when pleasure and intimacy was at it’s highest point and also it’s lowest point when you possibly felt like you were not aroused. He Zilbergeld states that within a sexual situation you must be comfortable, confident, and excited in order to get the fulfillment of an open experience. By clearing your mind of all the unnecessary chaos, you are able to transmit sexual messages that will result in better satisfaction when it comes to intimacy. 

In order to fully understand your condition, you and your partner must communicate all factors to understand sexual desires. By incorporating sexual enhancement programs, couples are able to make time for one another in having a better understanding of sexual fulfillment. “To sustain a loving sexual relationship, individuals must be willing to face the threats to the defense system that loving another person and being loved for oneself evoke” (Yarber, Sayad, Strong, 2015 p 455). When it comes to sexual difficulties, couple have the opportunity to cooperate with one another to help each other have a better intimate relationship. 

Differences in sexual desires amongst couples can tend to cause arguments which results in a negative way within the relationship. “One of the most significant elements of enhancing our physical experience of sex is intensifying arousal” Yarber, Sayad, Strong, 2015 p 459). Through homework exercises and programs, couples are able to better understand one another. From the readings you can see that these types of programs and exercises are somewhat comparable to sex therapy. Some couples may also incorporate using erotic aids such as sex toys to enhance sexual responsiveness. Having excellent sex is adaptable and varies with couples but the outcomes are able to be altered with the time and dedication to improving the experiences.

2.  Describe the major obstacles to gay, lesbian, bisexual, and transgendered (GLBT) people in obtaining adequate health care.

Lesbian, gay, bisexual, and transgendered persons face unique health care challenges.  Fear and costs are the main reasons the LGBT community faces challenges in obtaining adequate health care.  More than half of this population of Americans state that they have experienced violence, threats, or harassment because of their sexuality or gender identity.  LGBTQ people receive poor quality of care not only due to stigma, but also because of a lack of healthcare providers’ awareness as well as insensitivity to the unique needs of this community.  These discriminations in healthcare settings endanger LGBTQ people’s lives through delays or denials of medically necessary care. 

In many times, the patients want to talk about their sexual health but fear they would be discriminated against or stigmatized.  Because they don’t want discrimination to affect the quality of healthcare they receive, many people do no tell their doctors about their sexual orientation.  Because of this hesitation, lesbians and bisexual women have higher rates of breast cancer than heterosexual women since they have been found to get less routine health care than other women.  By eliminating LGBT health disparities and enhancing efforts to improve LGBT health are necessary to ensure that the LGBT individuals can lead long, healthy lives.  

Sexuality is an integral part of a patien’ts identity that needs to uniquely defined by each patient.  It cannot be determined simply by looking at someone.  Healthcare providers may lack adequate training on the specific needs and clalenges faced by the LGBT community.  Although there has been an uprising in LGBTQ-friendly healthcare providers, we as whole need to bring more awareness and help stop the discrimination, a lot of LGBT fear of the discrimination that they stop from seeking the needed medical care that they should have.  A way to improve this, is that healthcare providers can use inclusive language so that patients doesn’t need to feel that they have defend their sexual identity. The wrong words used by the healthcare provider can be experienced as heterosexist and can reveal a lack of understanding.
