Propose a change to one aspect of your local or regional health care system or program that would improve outcomes. Then, conduct a comparative analysis of other, non-U.S. health care systems, focusing on the proposed change. Summarize the proposed change and your comparative analysis in a 4-5 page report.

Health care systems around the world provide useful models for analysis. Familiarity with different models and approaches to health care enables leaders to identify what works and what does not, as the basis for proposing a change. As we continue to evaluate the complex and fragmented system in the United States, it is important for nurse leaders to become familiar with the programs and systems that provide evidence-based quality care that is affordable and focused on continual improvement.

This assessment provides an opportunity to examine a local or regional health care issue from a global perspective.

Preparation

Your organization, in collaboration with the key stakeholders from the community, is funding an initiative to investigate potential improvements in the local or regional health care system. As a nurse leader attuned to the effects of health care policy and finance on the provision of affordable, high-quality care, you have been asked to join the task force conducting the study.

You know that an examination of other countries’ health care systems can provide a solid, evidence-based foundation for evaluating outcomes and identifying benchmarks. Consequently, you have decided to undertake an analysis of selected, non-U.S. health care systems and compare them to each other and to the existing local or regional U.S. system to help inform decision making as the task force considers proposed changes.

In this assessment, you will propose a change to one aspect of your local or regional health care system or program. Conduct a comparative analysis of different health care systems and summarize your proposed change and findings from your analysis in a report to executive leaders. To prepare for your assessment, you are encouraged to begin thinking about the non-U.S. health care systems you might like to examine. In addition, you may wish to:

Course Project Milestone – Topic Selection and Discussion

 

Instructions
This assignment is the first step in a three-part project. You only need to focus on part one at this point. Each step will build on earlier steps. However, it is not a matter of providing a rough draft of all or even part of the entire project here in week three. That is, further steps might require completely new and original text.  At the same time, completing each step will aid you in completing a future step or future steps. And, you should use the same topic in all steps.

First, select a topic of moral controversy, debate, disagreement, and dispute, Examples of such topics are euthanasia, the death penalty, abortion, cloning, etc. You can pick any such topic. It need not be listed here.

Next, detail the positions of each side of the ethical debate. Note at least two moral reasons each side presents to show their view on the topic is correct.

Now, we want to evaluate these positions using the moral theories we studied this week:

  • What would an Ethical Egoist say about this topic? What side would the Ethical Egoist take? What would the Ethical Egoist say to justify their moral position? Is there a conflict between loyalty to self and to community relevant to your topic? If so, how so? Note what you feel is the best course of action.
  • What would a Social Contract Ethicist say about this topic? What side would the Social Contract Ethicist take? What would the Social Contract Ethicist say to justify their moral position? Does your topic involve a collision between personal obligations and national ones? If so, how so? Note what you feel is the best course of action.

Finally, reference and discuss any professional code of ethics relevant to your topic such as the AMA code for doctors, the ANA code for nurses, or any other pertinent professional code. State whether and how your chosen topic involves any conflicts between professional and familial duties.

Many assessment principles are the same for children and adults; however, unlike with adults/older adults, where consent for participation in the assessment comes from the actual client, with children it is the parents or guardians who must make the decision for treatment. Issues of confidentiality, privacy, and consent must be addressed. When working with children, it is not only important to be able to connect with the pediatric patient, but also to be able to collaborate effectively with the caregivers, other family members, teachers, and school counselors/psychologists, all of whom will be able to provide important context and details to aid in your assessment and treatment plans.

Marius is a third grade student who transferred to Oakwood Elementary School late in the fall. His teacher, Mrs. Pfirman, has noticed that he seems to struggle with many independent reading assignments. When Mrs. Pfirman administered the mid-year universal screening measure, she was not surprised to see that Marius’ reading score was below the grade-level benchmark. Consequently, Mrs. Pfirman decided to begin Level 1 RTI by monitoring Marius’ reading performance once a week for seven weeks using measures to assess his decoding, phonemic awareness, and vocabulary skills. Marius’ mother indicates that he was on grade level prior to moving; however, records received from his previous school do not show this. In other subject areas, Marius excels. His math skills are above the benchmark for his grade level. His speaking skills and verbal vocabulary are also on or above grade level.

On the initial assessment, the Beginning Phonics Diagnostic Assessment, Marius scored perfectly on all consonants and uppercase letters. He was able to sound out all short vowel letter sounds, and all vowel-consonant (VC) and consonant-vowel-consonant (CVC) words with beginning and continuous sounds. However, he was unable to read four of the six words on the measure of CVC words beginning with stop sounds, and only read one of the words on the consonant-consonant-vowel-consonant (CCVC) words with beginning blends sub-test. He read no other words on the rest of the assessment correctly.

On the Acadience Reading third grade beginning of the year assessment, Marius scored intensive. His scores on this assessment for weeks 5, 6, and 7 were 22, 27, and 33. The benchmark that would be expected after the 7 weeks of interventions is 30 and the expected criteria is 1.6. In addition, Marius’ rate of growth was 1.4.

On a Peabody Picture Vocabulary Test, he scored in the 84th percentile, which is well above average.

Marius is extremely interested in dinosaurs, Minecraft, and science. He has two dogs that he is extremely fond of and shares stories about these dogs in school. He is sometimes off task, pretending to be a dinosaur or a character from Minecraft. He knows just about every dinosaur by name, geographical region found and era. He knows what they all eat, how they lived, and when they were killed. He has seen all of the Jurassic Park movies (according to his self-report anyway) and is eagerly hoping to one day bring the dinosaurs back using DNA like they do in the movies.

Marius is from a single parent home and appears to move frequently. His mother does have a good job, though, and he appears to have other strong family ties, including some visitations with his dad. Marius is from a bi-racial couple who never married and do not live together, although they maintain a good parenting relationship.

Compose a 500-750 word evaluation of Marius incorporating the following:

Unit 6 assignment

Imagine that you are a defense attorney hired to defend a client who is charged with Reckless Homicide as a result of a traffic crash. As you review the discovery materials provided by the prosecution, you learn the facts upon which the prosecution will rely at trial. Keep in mind that these are the facts as contained in police reports, not necessarily what your client states transpired just prior to the crash. You learn that police state that your client was driving his NEW motor home, along with his wife, headed to a weekend getaway, when a terrible accident occurred with a truck, rendering the motorhome not drivable. The driver of the truck was killed in the crash.However, your client’s motorhome was towed to a secure police evidence lot, as a result. No arrests were made at the time, although the investigation continued after the crash. You learn from the police reports that the motorhome was equipped with cameras which record both the driver of the motorhome while driving and also what the driver’s view would be in a forward direction from the driver’s seat. Police determine that having the images from those cameras would assist in determining whether your client was at fault for the crash which killed the other driver, as well as provide clarity as to how the crash occurred. Police extract both of the cameras from the motorhome, submit them for analysis, and subsequently claim that the images resulting from the analysis of the cameras supported probable cause to arrest your client. He was arrested shortly thereafter for Reckless Homicide of truck’s driver. Police did not obtain a search warrant prior to retrieval of the cameras.As the defense attorney, you must now determine if there is a basis to suppress evidence in this case, specifically, the motorhome’s cameras and mages retrieved from them.In 2-3 , submit  answering each of the following questions. You may not be old enough to remember the Maybelline Make-up commercial that stated: “Maybe she’s born with it. Maybe it’s Maybelline,” when determining if the model’s beauty was natural or created by the make-up (Noonan 2014). This seemingly innocent commercial brings up the question of whether we are a product of our nature or our nurture. This commercial asked if she was born beautiful or made beautiful. This same question can be applied to many things.

Address these questions:

  • Is a person born a genius or made a genius through education and hard work? We call a talented musician “gifted.” Does that mean the individual was born with a natural gift for music (nature) or did they just work harder than other people to be that good (nurture)? Explain your reasoning. Would it make a difference in your answer if that musician were nine years old or thirty years old? In what way?
  • This is all part of the decades old debate over whether we are born with certain innate personality characteristics and abilities (our genes), or whether we learn and are impacted by our experiences to be the person we become (environment).
  • Do they work independently – or do they work together? Explain your choice and find a source to support your view on this.
  • Probably one of the most famous answers to this age-old question comes from neuropsychologist Donald Hebb. When a reporter allegedly asked Hebb which was more important – nature or nurture, he replied, “Which contributes more to the area of a rectangle, its length or its width”(as cited in Meaney, 2004. p. 2)?
