Traditionally, performance reviews are completed using preset performance appraisal forms or templates. An appraisal form should have detailed framework for measuring performance. Whether you are an employee, HR representative, or manager, you will have to complete and/or review performance appraisals. For this discussion, you will create a performance appraisal template that you believe would benefit the organization you currently or have previously worked for. Prior to beginning work on this exercise, read the article Viewpoint: How to Redesign Your Performance Appraisal Template (Links to an external site.). Additionally, review several performance appraisal templates available on the Internet.

Part 1

Create a performance appraisal template that you feel meets the needs of your current or previous position and organization. The appraisal should include a rating scale and five competencies you would recommend the organization to evaluate staff on. At minimum, include a rating scale guideline (i.e., description of what each rating is composed of). Attach your performance appraisal template to the initial post.

Part 2

In the body of your post, explain why you selected the five competencies and how your selections contribute to effective employee performance management, training, and development programs.

Your initial post must be a minimum of 300 words. Cite at least one scholarly source to support your response.

The Future of Health Care

Fundamental shifts in innovation tend to occur in seven-year cycles. And while it’s possible to foresee what could happen over the next one or two cycles, looking ahead to 2040 (three cycles from now) will likely reveal a completely different reality than what we can comprehend today. Watch the video to see what one company believes will emerge in the next 21 years that will dramatically reshape the health care industry.

Step 3:  Browse the Internet for web pages, journal articles, news stories, speeches, or videos that relate to the future of the health care industry.  Write a short summary of one of these, then share your opinion on the article and subject matter.  Your post should be at least 350 words and should be written in fully formed paragraphs.  Make sure to adhere to the rubric, which can be found at the end of this assignment.

Discussion: Building a Health History

Effective communication is vital to constructing an accurate and detailed patient history. A patient’s health or illness is influenced by many factors, including age, gender, ethnicity, and environmental setting. As an advanced practice nurse, you must be aware of these factors and tailor your communication techniques accordingly. Doing so will not only help you establish rapport with your patients, but it will also enable you to more effectively gather the information needed to assess your patients’ health risks.

For this Discussion, you will take on the role of a clinician who is building a health history for a particular new patient assigned by your Instructor.

To prepare:

With the information presented in Chapter 1 of Ball et al. in mind, consider the following:

  • By Day 1 of this week, you will be assigned a new patient profile by your Instructor for this Discussion. Note: Please see the “Course Announcements” section of the classroom for your new patient profile assignment.
  • How would your communication and interview techniques for building a health history differ with each patient?
  • How might you target your questions for building a health history based on the patient’s social determinants of health?
  • What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
  • Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration.
  • Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the Seidel’s Guide to Physical Examination text, or another tool with which you are familiar, related to your selected patient.

16-year-old white pregnant female living in an inner-city neighborhood

  • How would your communication and interview techniques for building a health history differ with each patient?

     The interview process has to be adjusted pending on the age group, gender, financial status, culture, and language barrier of the individual being assessed. One must provide a private area and be close to the patient during the interview to provide comfort. Use of laymen’s terms helps the individual to understand healthcare information clearly. Making eye contact is especially important unless there is a cultural factor involved. Taking notes should be minimal and if using a computer; it should be positioned so the patient can see the screen as well. Introduce one’s self to the patient and others present and use their names to build a friendly trustworthy environment.

     When speaking with my adolescent patient I would emphasize that our discussion is confidential with limitations if there is a danger to her or others. We will speak privately unless she wants someone else present. She will be given the opportunity to discuss whatever topics she feels comfortable discussing.

  • How might you target your questions for building a health history based on the patient’s social determinants of health?

     There are several areas to concentrate on to build a history based on social determinants of health. One should enquire about the individual’s personal status, sexual history, living conditions, occupation, environment, military record, and social needs.

     According to the information provided my patient lives in an inner city neighborhood. This may be an indication of poverty and limited resources. One would inquire if housing, clothing, food, and access to medical care are adequate. If they are not, community resource information would be provided and a social worker referral to aide her. Also, one must ascertain if she is in an abusive relationship or being abused at home.

  • What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?

     Assessing an individual’s health risks is very important in determining what potential health problems one may face. “One of the main goals of documenting the social history (SH) of the patient is to identify factors outside of past or current medical conditions that may influence the patient’s overall health or behaviors that create risk factors for specific conditions” (Sullivan, 2018). Asking what their eating habits are, do they exercise, how much alcohol do they consume, do they smoke cigarettes, do they use recreational drugs, what are their sleeping habits, do they take their medications as prescribed, do they practice unsafe sex, and what is their family history of medical problems. Also, inquiring about their family history of mental illness is important. Religion and culture can affect how the individual’s treatment plan will be developed.

    One would obtain the young lady’s family health history at this time to determine if she is at risk for familial issues during her pregnancy. This would be a great opportunity to provide prenatal education and stress the importance of continuity of care during pregnancy.

  • Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration.

     Adolescent girls are more likely to die during childbirth than women in their twenties or older.  If a pregnancy is unplanned, the mother may not be getting the prenatal care she and her baby need or may not even be healthy enough to carry a child to term.

  • Teens are often unprepared for the realities involved in parenting an infant. Often, complex relationships and financial burden combined with balancing school and parenting are stressful and can put a newborn at risk.
  • Teens who are pregnant or raising a baby have a hard time finishing school. Only 3 percent of teens who have a baby receive their college diploma before the age of 30.
  • Many teen parents are single. Being a single parent can have financial and emotional stressors and a stressed parent puts a baby at risk.
  • Parents often need resources to help them navigate their child’s well-being and development. Teens may not be aware of this type of assistance. (Heuristic, 2016)

Changing business structure

Jones and Turner are interested in possibly changing the structure of their business to an S Corporation and would like to know more about this entity. They are excited by the fact that the S Corporation is a “flow through entity” and the fact that they don’t pay income tax.

They also heard that if they pay themselves dividends as S Corporation shareholders, they won’t have to pay any Social Security or Medicare tax (payroll taxes).

Digital Transformation Management

I have an assignment due on the 9th of  December for the module digital transformation management its about carrying an analysis  of an organisation (private or public) of your choice and to provide an outline for its Digital Transformation. The organisation selected must be of such a suitable size that it merits analysis of a strategic transformational nature and it should potentially benefit from a digital transformation (e.g., an SME, a division of a multinational corporation, a small or medium-sized public organisation, or a division of a large public organisation). The analysis of the organization and the outline for its Digital Transformation should be guided by the themes below.

You should structure your submission as follows:
(a) An introduction to the report (c. 100 words)
(b) The main section where you address the required themes which are described in detail below (c. 2,800 words).
(c) Conclusion (c. 100 words)
(d) Reference list
(e) Appendices (if required).

In the Introduction to the report you should explain the rationales for choosing the organisation, the main aims of the report, and main theoretical framework(s) (e.g., a specific systems theory, neo- Schumpeterian theory) you rely on. Explain briefly the rationale for selecting a specific systems theory framework (e.g., activity theory) for your analysis. In the main section you should apply, where relevant, concepts, models and theories covered in the module, and from the digital technologies and transformation literatures in general. These concepts, models and theories should be used to analyse the specific organisation you have chosen and to provide an outline of its Digital Transformation.

It is recommended that you use the Harvard approach to citation and referencing appropriate for the Social Sciences (see the library website, which provides information about the Harvard style, citing Chinese sources etc.). Furthermore, it is recommended that you provide an end list of references and do not adopt other forms of referencing (e.g. in footnotes).

Appendices and references are not included in the word count. Appendices are regarded as supportive but not essential to the submission and therefore are not marked.

Themes to be addressed in the report
The main section of the report should address the following THREE themes:

1. The systems viewpoint to the organisation before the digital transformation

Analyse the organisation from a systems viewpoint (e.g. by using activity theory) with a focus on the period before the major digital transformation.

  1. (a)  An introduction to the organisational context and to the nature of its business. Do not
    assume your reader will know about the organisation. If this has not been given in the introduction section, you should include sufficient information such as the purpose of the organisation, its structure, size and location, to enable the reader to understand your analysis.

(b)  An overview of the organisation from a systems viewpoint, with a focus on understanding the interplay of digital technologies and other main tools, and of managerial and organisational factors (using one of the models covered in the module, if possible). Focus on those parts of the organisation that are relevant for a possible digital transformation process. Include external actors or organizations that affect the transformation of the organisation.

2. The factors driving the digital transformation

Analyse the factors that drive the digital transformation of the organisation using the concepts, models and theories covered in the module, and recommended wider reading.
Include into this analysis:

  1. (a)  An examination (for example, by using systems theory) of the key problems facing the organisation (before the digital transformation), and how digital transformation could help address these problems. Assess whether these key problems arise from the prevalence old technologies, old management models/business models/organisational paradigms, the changing context (e.g., new rivals, new regulations) and other key factors.

  1. (a)  Critically evaluate the current state of digital transformation (what has been transformed and what not) in your organisation—you can build on the analysis conducted in prior sections.
  2. (b)  Suggestthepossibleoutcomeofacomprehensivedigitaltransformation.Thesuggestions should adopt a systems viewpoint with a balance between the use of novel digital technologies and management/organisational aspects, and should be supported by a sound argument why this outcome represents a solution that benefits the organisation.
