Discuss the questions that would be important to include when interviewing a patient with this issue

Kayla is a 16-year old female who presents with complaints of pelvic pain and previously diagnosed with pelvic inflammatory disease. When interviewing an adolescent presenting with these symptoms, it is crucial to ask questions related to social history in privacy (Smorgick & As-Sanie, 2018). Such questions should cover the home situation, school and other activities and potentially sensitive domains of substance abuse, sexual activity, depression, eating behaviors, and safety. In particular, it would be important to ask whether she had experienced any sexual assault in school or at home and if any, whether she noticed any bleeding during the assault. Another important question to ask is whether she has noticed any discharge and the color of the discharge. A normal vaginal discharge is clear and usually odorless. However, changes in the color, amount, or smell of the vaginal discharge may indicate a problem. For example, a white discharge that is clumpy and looks similar to cottage cheese may be a sign of yeast infection, which may cause symptoms such as burning on urination or vaginal itching. Similarly, a yellow/green discharge can be a sign of STIs or bacterial infection, and which can cause pain in the pelvic (Stratton, 2019).

Describe the clinical findings that may be present in a patient with this issue

The most common symptom of pelvic inflammatory disease is pain or tenderness in the stomach or lower abdomen. Other symptoms include abnormal vaginal discharge, usually yellow or green with an unusual odor, fever, tiredness, pain and bleeding during sex, burning sensation upon urination, and pain in the upper abdomen. Other clinical findings include tenderness of fallopian tubes and ovaries and tenderness of the cervix.

Are there any diagnostic studies that should be ordered on this patient? Why?

There are several diagnostic studies that should be ordered on this patient. A pelvic exam should be ordered first to examine her reproductive organs and look for signs of infection. Vaginal culture can also be taken to check possible bacteria, while the urine test may help to rule out urinary tract infection, which causes similar pelvic pain. Also, the doctor could order for an ultrasound to get clearer images of the reproductive system or CT scans to help detect abnormal structures or growth. Another diagnostic study to be ordered is laparoscopy to view the pelvic organs and check for abnormal tissues or signs of infection.

List the primary diagnosis and three differential diagnoses for this patient. Explain your reasoning for each

Primary diagnosis – Pelvic inflammatory disease – The patient is sexually active and admits to having multiple sexual partners without using a condom. Besides, she has admitted pain and tenderness in the lower abdomen.

Differential diagnoses

Urinary tract infection – Having multiple sexual partners and sometimes without a condom could have caused urinary tract infection.

Ovarian torsion – The patient has come with symptoms of severe pelvic pain that she rates as 10 out of 10.

Endometriosis – The patient admits abdominal cramping and pain in the lower abdomen and pelvis.

Discuss your management plan for this patient, including pharmacologic therapies, tests, patient education, referrals, and follow-ups.

The main goal in the management plan of this patient is to alleviate symptoms and improve her quality of life. The patient needs some pain relievers such as ibuprofen or acetaminophen to provide partial relief from her pelvic pain. The patient may also need some antibiotics to help in case of any infection. In addition, the patient may be prescribed antidepressants to help improve the pelvic pain. Also, the patient needs to be educated about the benefits of practicing safe sex, particularly using condoms every time she has sex and to limit the number of sexual partners (Sanfilippo et al., 2019). She should also be advised to make an appointment for regular screening of sexually transmitted diseases and request her partners to be also tested. Also, the patient can be referred to physical therapists to help her develop coping strategies for the pain. Such include stretching exercises, massage, and other relaxation techniques.

Breach of Contract

Johnny, a neighbor who is not a merchant under the Uniform Commercial Code, offers to buy a car from Mark for $30,000.   Mark asks Johnny for some time to think about it. Johnny says sure. He writes on a piece of paper that he will keep the offer open for two weeks.  
A week later Johnny sees another car he would rather buy. He purchases that, and then he tells Mark that he is revoking his offer. 
Two days after that Mark said: “I’m sorry Johnny you made an offer in writing to buy my car. I’m going to hold you to that.”   
Johnny replied: “Sorry I cannot do that.  But I will promise to pay you $10,000 for the help you gave me last year around the house.”   Somewhat mollified Mark accepts. 
A week later and Johnny decided to renege on that promise as well.

Ethics play a critical role in the practice and research of criminal justice. Criminal justice is about safety and ensuring individual rights at the same time. It is incumbent upon those that practice criminal justice and conduct criminal justice research, to do no harm. When it comes to research, ethical concerns are paramount. The moral process in research is based on the law and principles outlined in the Belmont Report. The three main principles of the report are respect for persons, beneficence, and justice. Respect for persons means that those in the study or research do so of their own free will. Beneficence means that the research does no harm to the participants and seeks to do good. Justice implies that research’s benefits and burdens are fairly distributed (Maxfield & Babbie, 2018).  According to Vito and Higgins (2015), ethics covers both how the research is done and the researcher’s actions. Research and the application of programs should be based on science and fact, not emotion and assumptions. The Cambridge-Somerville Youth Study, Court Volunteers, and Scared Straight are examples of presumption of good that were wrong (McCord, 2003).

The concept of not causing harm and being personally neutral in judgment, a cornerstone of research and criminal justice is well documented in the Bible.

You shall do no injustice in judgment. You shall not be partial to the poor, nor honor the person of the mighty. In righteousness, you shall judge your neighbor. You shall not go about as a talebearer among your people; nor shall you take a stand against the life of your neighbor; I am the Lord. (New King James Version, 1982, Leviticus 19:15-16)      

Leviticus clarifies that those involved in criminal justice research should have no preconceived notions about individuals or subjects and that having such ideas would be an injustice.  Indeed, Nehemiah makes the point that folks who need to be helped have already suffered and that those entrusted to help them should not add to their burden or add hardship to them.

Yet now our flesh is as the flesh of our brethren, our children as their children; and indeed, we are forcing our sons and our daughters to be slaves, and some of our daughters have been brought into slavery. It is not in our power to redeem them, for other men have our lands and vineyards. And I became very angry when I heard their outcry and these words. After serious thought, I rebuked the nobles and rulers and said to them, “Each of you is exacting usury from his brother.” So, I called a great assembly against them. (New King James Version, 1982, Nehemiah 5:5-7).

Indeed, even the officer on the street is bound by these conditions of not doing more harm. The concepts highlighted in the Bible can be seen through Supreme Court decisions such as Tennessee v. Garner when the Court ruled that deadly force can only be used if the officer making the arrest faces the threat of death or serious bodily injury (Tennessee v. Garner, 1985). In Graham v. Connor, the Court said that in any arrest, the force used must only be the amount necessary to make that arrest (Graham v. Connor, 1989). In both these cases, it is obvious the Court is following a Christian View in telling police that they cannot add to the harm and only do what is necessary to make an arrest. Whether practitioners or researchers do not harm is a required and ethical standard.

Aligning Risks, Threats, and Vulnerabilities to COBIT P09 Risk Management Controls

  Introduction

Ask any IT manager about the challenges in conveying IT risks in terms of business risks, or about translating business goals into IT goals. It’s a common difficulty, as the worlds of business and IT do not inherently align. This lack of alignment was unresolved until ISACA developed a framework called COBIT, first released in 1996. ISACA is an IT professionals’ association centered on auditing and IT governance. This lab will focus on the COBIT framework. The lab covers two released versions: COBIT 4.1, which is currently the most implemented version, and COBIT 5, which was released in June 2012. A newer version, COBIT 2019, was released in 2019.

Because COBIT 4.1 is freely available, with registration, at the time of this writing, the lab uses this version to present the handling of risk management. COBIT presents this topic using a set of COBIT control objectives called P09. COBIT P09’s purpose is to guide the scope of risk management for an IT infrastructure. The COBIT P09 risk management controls help organize the identified risks, threats, and vulnerabilities, enabling you to manage and remediate them. This lab will also present how COBIT shifts from the term “control objectives” to a set of principles and enablers in later versions.

In this lab, you will define COBIT P09, you will describe COBIT P09’s six control objectives, you will explain how the threats and vulnerabilities align to the definition for the assessment and management of risks, and you will use COBIT P09 to determine the scope of risk management for an IT infrastructure.

Chief complaint: “I’m here for a medication refill because I ran out of my medicines”.

HPI:  Mrs. Allen is a 68-year-old African American who presents to the clinic for prescription refills. The patient indicates that she has noticed shortness of breath which started about 3 months ago. The SOB gets worse with exertion, especially when she is walking fast, and it is resolved when she is resting. She reports that she is also bothered by shortness of breath that wakes her up intermittently during her sleep. Her symptoms of shortness of breath resolve after sitting upright on 3 pillows. She also has lower leg edema pitting 1+ which started 2 weeks ago. She indicates that she often feels light headed at times with intermittent syncope episodes while going up a flight of stairs, but it resolves after sitting down to rest. She has not tried any over the counter medications at home.

She started taking her medications, but failed to refill the prescriptions because she cannot afford the medications as she only works part-time and lives alone. In addition, she reports that she does not think taking all these medications would help her condition anyway.

Questions:
