Informed consent is obtained after a client receives complete disclosure of all pertinent information provided by the provider regarding the surgery or procedure to be performed. The nurse may obtain informed consent only after the client verbalizes understanding of the potential benefits and risks associated with the surgery or procedure.

Elements of Informed Consent:

  1. The individual giving consent must fully understand the procedure that will be performed, the risks involved, expected/desired outcomes, expected complications/side effects, and alternative treatments or therapies available.
  2. Consent is given by a competent adult, legal guardian or designated power of attorney (DPOA), emancipated or married minor, parent of a minor, or a court order.
  3. A trained medical interpreter must be provided when the person giving consent is unable to communicate due to a language barrier.
  4. The nurse’s role is to witness the client’s signature and ensure the provider gave the necessary information and that the client understood and is competent to sign. If it has been identified that the client does not understand the procedure or is not competent to sign the informed consent, the nurse must STOP the process and the physician must be notified immediately.

A consultation occurs when a professional provides expert advice in a particular area and determines what treatment or services the client requires. The nursing intervention evolves around facilitating coordination with other health care providers throughout the continuum of care in order to protect the client from conflicting and potentially dangerous treatments.

A referral is a formal request for a special service by another care provider so that the client can access the care identified by the primary care provider or consultant. Some examples of referrals include physical therapy, wound care, and hospice. The nursing intervention continues to evolve around monitoring the client’s response and progress.

Directions: There are three scenarios that will be used in this discussion posting. You are all responsible for the initial discussion posting and then responding to one posting in the remaining scenarios. 30-year-old male presents to the emergency department with RLQ pain and fever. Labs reveal an elevated white blood count. The surgical intern tells the patient that appendicitis is suspected and that the plan is to proceed with surgery to remove the appendix. He explains that the surgery is to be performed to prevent complications of appendicitis, including abscess formation, sepsis or even death. He goes on to explain the risks of surgery, including bleeding, infection and damage to adjacent organs. What additional information needs to be included in this consent? Provide an evidence-based reason for your answer.

14-year-old girl presents to the emergency department, accompanied by her 10-month-old daughter and boyfriend. The boyfriend is the child’s father. No cause of the pain is able to be delineated, and the plan is to take the girl to the operating room to perform a diagnostic laparoscopy. In this case, from whom should informed consent be obtained? George is a 70-year-old patient who has been assigned to you as his community-based nurse case manager. 1) Overall, would you say that group members’ responses closely correspond to or differ noticeably from the material about adolescent behavior that is presented in the textbook, lectures and video? Use examples to explain your assessment.

2) What factors at various levels of the ecology in Bronfenbrenner’s Bioecological Model of Development contribute to the behavior that is perceived to occur frequently among teens? Provide examples to illustrate the factors that you identify; for these examples,  draw upon the your fellow group members’ posts about their “risky and rebellious adolescent behaviors (or lack thereof) if you can; if you cannot (for example if your fellow group members who have posted before you have “used them up” so that ours would be repetitive, use examples from other adolescents that you have known. Nursing 530 discussion

Mr. B, a 40-year-old avid long-distance runner previously in good health, presented to his primary provider for a yearly physical examination, during which a suspicious-looking mole was noticed on the back of his left arm, just proximal to the elbow. He reported that he has had that mole for several years, but thinks that it may have gotten larger over the past two years. Mr. B reported that he has noticed itchiness in the area of this mole over the past few weeks. He had multiple other moles on his back, arms, and legs, none of which looked suspicious. Upon further questioning, Mr. B reported that his aunt died in her late forties of skin cancer, but he knew no other details about her illness. The patient is a computer programmer who spends most of the work week indoors. On weekends, however, he typically goes for a 5-mile run and spends much of his afternoons gardening. He has a light complexion, blonde hair, and reports that he sunburns easily but uses protective sunscreen only sporadically.

Physical exam revealed: Head, neck, thorax, and abdominal exams were normal, with the exception of a hard, enlarged, non-tender mass felt in the left axillary region. In addition, a 1.6 x 2.8 cm mole was noted on the dorsal upper left arm. The lesion had an appearance suggestive of a melanoma. It was surgically excised with 3 mm margins using a local anesthetic and sent to the pathology laboratory for histologic analysis. The biopsy came back Stage II melanoma.

  1. Describe the background of the problem. Tell the story of the issue and why it deserves attention.
  2. Identify the stakeholders/change agents. Who or what organizations  are concerned about, may benefit from, or are affected by this proposal.  List the interested parties, patients, students, agencies, Joint  Commission, etc.
  3. Provide the PICOT question. (PICOT:  Population-Intervention-Comparison-Outcome-Timeframe). Make sure that  the question fits with your graduate degree specialization.

The objective should address what is to be gained.  This is a restatement of the question, providing focus. Measurements  need to be taken before and after the evidence-based practice is  introduced to identify the expected changes.
  5. Provide supportive rationale that the problem or issue is an  important one for nursing to resolve using relevant professional  literature sources.

Develop an initial reference list to ensure that there is adequate  literature to support your evidence-based practice project. Follow the  “Steps to an Efficient Search to Answer a Clinical Question” box in  chapter 3 of the textbook. Refer to the “Search Method Example” as the  format in which to compile this data.

  1. The majority of references should be research articles. However,  national sources such as Centers for Disease Control and Prevention  (CDC), National Center for Health Statistics (NCHS), Department of  Health and Human Resources (HHS), or the Agency for Healthcare Research  and Quality (AHRQ) and others may be used when you are gathering  statistics to provide the rationale for the problem.
