Forensic scenario two: Mr. M. (not guilty plea) 

Several things stuck out at me in Mr. M’s case study.  the first thing was his reckless behavior trying to avoid his capture, especially ramming the police car.  Next, what really jumped at me was the hospital staff’s concern for drug or alcohol use, when was proven incorrect.  The staff describe his behavior as essentially erratic to the point that he required a sedative.  What stuck out the most to me though is his sudden change of personality about four months prior to the incident which he is appearing in court for.  At the time of the personality change Mr. M lived with his parents and because extremely agitated and paranoid according to them.  Also noteworthy is that these claims were backed up by his employer.  Inf act, at the time of the incident Mr. M was headed home. 

Due to the sudden onset of symptoms, my first recommendation would be to have an MRI (magnetic resonance imaging) to check for any tumors of the brain.  the MRI test is the best way to rule out a physical issue and is highly tested and validated in the medical field.  the MRI would relate most to neuropsychology which according to Gregory (2014) has a direct bearing on the issue of pleading guilty by reason of insanity.  Should no tumor be found more psychological testing will be necessary.  I would also recommend the Rogers Criminal Responsibility Assessment Scales (R-CRAS).  This assessment measures reliability, organicity, psychopathology, and cognitive and behavioral control while assessing both psychological and situational factors at the time the crime was committed (Gregory, 2014). 

According to the APA’s ethical Principals one should not use assessments which are outdated, or under validated.  Both the MRI (which is relevant because of the effects a brain tumor could have on one’s personality) and the R-CRAS have changed their technicalities to stay appropriate to the times.  They also have been validated by use and are very common tests.  In particular, the MRI is also used for other purposes and has a high success rate in finding issues such as tumors, or blood clots (another possibility depending upon placement) within the brain.  An MRI is a type of neuropsychological assessment, a topic discussed in Gregory, 2014.  This text explains that this type of assessment provides data which is valid, and objective for the judge when trying to merit such a defense as not guilty by reason of insanity.  According to APA (2013) guidelines if ordered by the court even if the defendant does not want to comply with such tests as these the assessments can still be done.  This validates the importance of these types of tests in cases where a person is trying to claim innocence based upon a plea of insanity.

The Assignment

Develop a Focused SOAP Note, including your differential diagnosis and critical-thinking process to formulate a primary diagnosis. Incorporate the following into your responses in the template:

  • Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
  • Objective: What observations did you make during the psychiatric assessment?  
  • Assessment: Discuss the patient’s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses with supporting evidence, listed in order from highest priority to lowest priority. Compare the DSM-5 diagnostic criteria for each differential diagnosis and explain what DSM-5 criteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case.
  • Plan: What is your plan for psychotherapy? What is your plan for treatment and management, including alternative therapies? Include pharmacologic and nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan. Also incorporate one health promotion activity and one patient education strategy.

Accountability in Nursing Profession:-

Nursing Accountability is an ethical code of conduct where nurses are responsible for their actions (ANA 2015). Professional accountability impacts patient care through health promotion, patient teaching, counseling, daily life activities, and collaboration with the interdisciplinary team (physiotherapist, dietician, pharmacist, etc.) (Leonenko and Drach‐Zahavy, 2016). The nurses who are accountable focus on patient’s trust in the nursing profession. When the nurses work together by improving patient care as a team, they can earn the patient’s trust. Accountability is seen in nurses by following practice guidelines of the nursing practice act, questioning the standards of care to the provider’s order, maintain confidentiality, ensuring safe quality care to the patient, and delegating works in team members to patient care. (Murphy, 2020)

In daily lives, nurses are accountable for the work they do in clinical. The nurse should be responsible to self, peers, employer, and patient. Examples of professional accountability to self in nursing would be nurses working on unsafe nursing practices, like if the unit is overload with high acuity patients and more work overload. A nurse should be responsible for refusing the work in an unsafe setting to protect the public and be accountable. The nurses should make sure that organizational policies and refuse to work in a bizarre environment. If the nurse cannot take responsibility for the vast number of patients, it is better to discuss with the authority in time rather than face the consequences due to lack of care. (Murphy, 2020)

An example of evidence-based nursing accountability practice will be my personal experience working in a nursing home if a new patient comes in admission on my time. I am accountable for the patient’s activity. So, correct documentation about the patient would be necessary to justify my accountability. Likewise, I need to update the legal information, ensure the patient signs the admission and treatment consent form before administering any meds, or providing any care. If I gave any medicine as the patient requested without a prescription, I would be the one who has to justify my action. I have to check before administering meds to the patient; without knowing the history and known allergies, I cannot help the patient giving medicine. It is beyond nursing practice. I will be the accountable one. So, as a nurse, I should always follow the nursing process to take care of the patient. I have to assess the patient by taking vitals and physical assessment. Secondly, planning regarding the care and treatment is significant for the patient. Thirdly, I have to implement the care provided to the patient. Fourthly, I had to give a rationale for the implementation of care, and lastly, evaluation helps know the patient’s outcome. Documentation and reporting have a significant role in professional accountability.

Assignment Instructions:  The Course Project for this class is divided into three major Assignments, 1) Annotated Bibliography, 2) Outline and 3) Final Presentation. The first part is the Annotated Bibliography. An annotation is a summary and evaluation, and your annotated bibliography will include a summary and evaluation of some of the sources (or references) you will use for your presentation.

To prepare for this assignment, I recommend that you do the following:

  • Read these directions carefully.
  • Review the sample annotated bibliography provided to you below. 
  • Read the grading criteria below.  The grading criteria is a detailed evaluation that I will use to assess your performance.  It also will help you understand what is expected of you as you prepare your assignments.

The reason the annotated bibliography is included as part of the research project is that writing an annotated bibliography is important in that it provides excellent preparation for the final presentation. One of the issues regarding any type of research, especially in the biosciences, medical, health/nutrition and exercise/fitness fields, is the credibility of the sources used, particularly those obtained from various websites. Freud and Rogers: Comparing Takes on Modern Life

 

  • Read the following background on the time periods and societies in which Freud and Rogers lived.
    • Sigmund Freud lived in a time of change that included a catastrophic world war that set the stage for an even bigger world war. Ten years after Freud’s birth in 1856, Austria went to war with Prussia in Germany. The result was the formation of the Austro-Hungarian Empire, which would expand and then disintegrate in the next 50 years. Freud was born into a wealthy, Jewish family and lived most of his life in Vienna, the Austrian capital. He would have been fully aware of the forces that marked profound changes in the 19th and early 20th centuries. He saw nationalist movements that destabilized the Austro-Hungarian Empire. World War I in 1914 was triggered by the assassination of Archduke Franz Ferdinand, the heir to the Austro-Hungarian throne. Consider that of the 7.8 million Austro-Hungarian forces that fought in the war, 90 percent (7.02 million) were causalities—killed, wounded, missing, or taken prisoner. (See the Optional Resource, WWI Casualty, and Death Tables, for these and additional statistics.) Freud also witnessed the rise of communism and fascism in Europe. When he died on September 23, 1939, Nazi Germany had invaded Poland and World War II had begun. As a Jew, he had left Vienna for England, where he died, to escape the Nazi threat. In comparison to the technology in the modern coffee shop, photography developed greatly in his lifetime, and the telephone was invented. But there were no computers or Internet or anything close to them. Telephones were not portable. And while there were hand-held cameras, they were nothing like the cell phone features of today.
    • The Learning Resources on Carl Rogers provide background on his life. Note that he spent much of his early life living on a farm in the U.S. Midwest—the opposite environment from Freud’s urban setting in a major European capital—and initially went to college to study agriculture and then the ministry before becoming a psychologist. Born at the start of the 20th century in 1902, Rogers witnessed tremendous change and development in his lifetime. He was 14 when World War I began and 37 at the start of World War II, from which America emerged as a major world power. He witnessed the Nuclear Age and its arms race, and the Cold War between the U.S. and the Soviet Union for nearly 50 years following World War II. He also saw the Civil Rights Movement and the Women’s Movement in the United States. In terms of technology, when he died in 1987, there were digital cameras, mobile phones, and laptop computers, although not of the convenient size, speed, and multiple features of current devices. The Internet was in primitive use, although social sites like Facebook had not yet been founded.
