 R.H. is a 74-year-old black woman, who presents to the family practice clinic for a scheduled appointment. She complains of feeling bloated and constipated for the past month, some-times going an entire week with only one bowel movement. Until this episode, she has been very regular all of her life, having a bowel movement every day or every other day. She reports straining most of the time and it often takes her 10 minutes at a minimum to initiate a bowel movement. Stools have been extremely hard. She denies pain during straining. A recent colonoscopy was negative for tumors or other lesions. She has not yet taken any medications to provide relief for her constipation. Furthermore, she reports frequent heartburn (3–4 times each week), most often occur-ring soon after retiring to bed. She uses three pillows to keep herself in a more upright position during sleep. On a friend’s advice, she purchased a package of over-the-counter aluminum hydroxide tablets to help relieve the heartburn. She has had some improvement since she began taking the medicine. She reports using naproxen as needed for arthritic pain her hands and knees. She states that her hands and knees are extremely stiff when she rises in the morning. Because her arthritis has been getting worse, she has stopped taking her daily walks and now gets very little exercise.

  1. In your own words define constipation and name the risk factors that might lead to develop constipation. List recommendations you would give to a patient who is suffering from constipation. You might use a previous experience you might have.
  2. Based on the clinical manifestations on R.H. case study, name and explain signs and symptoms presented that are compatible with the constipation diagnosis. Complement your list with signs and symptoms not present on the case study.

Endocrine Function:
C.B. is a significantly overweight, 48-year-old woman from the Winnebago Indian tribe who had high blood sugar and cholesterol levels three years ago but did not follow up with a clinical diagnostic work-up. She had participated in the state’s annual health screening program and noticed that her fasting blood sugar was 141 and her cholesterol was 225. However, she felt “perfectly fine at the time” and could not afford any more medications. Except for a number of “female infections,” she has felt fine until recently. Today, she presents to the Indian Hospital general practitioner complaining that her left foot has been weak and numb for nearly three weeks and that the foot is difficult to flex. She denies any other weakness or numbness at this time. However, she reports that she has been very thirsty lately and gets up more often at night to urinate. Student Goals and Intervention Plan

Teachers must work collaboratively with colleagues, school staff, and parents to develop and implement behavior plans for students engaging in challenging behaviors. All stakeholders need to have a clear understanding of the plan and the steps to take to ensure the student is learning behaviors that are more appropriate. Each member of the team must also know how to implement the consequences when/if a student does not engage in appropriate behavior. In some cases, behavior can be severe; all members of the team need to be aware of how to respond to a student in crisis.

Review the scenario below to inform the assignment:

Michael is a seventh grader who is in his first year of rotating from class to class. Michael has shown that he really enjoys his homeroom teacher, Mrs. Thomas, who also happens to be the special education teacher. Mrs. Thomas takes Michael out of his reading class, for half of the class time, to provide his reading minutes according to his IEP. While Michael is serviced in reading only, Mrs. Thomas will take Michael out during all testing to provide a quiet, more 1:1 environment for testing. All of Michael’s teachers are male aside from Mrs. Thomas.

Mrs. Thomas was called into a team meeting to discuss Michael’s behaviors in his other classes.

All of the other teachers reported during the meeting that Michael is disruptive in class. His social studies teacher has noted that Michael is redirected at least three times each class period for talking with other students or being out of his seat. His social studies teacher also noted that Michael enjoys talking about baseball and video games with his classmates rather than participating in class or completing tasks. When redirected a few times last week, Michael asked to use the restroom and was out of the classroom for at least 15 minutes each time.

Michael’s math teacher also reported that periodically Michael shouts out during class or talks with others during instruction and independent work time. The math teacher has tried to redirect his behavior, but like his social studies teacher mentioned when redirected, Michael will ask to use the restroom or see the nurse and remain out of the classroom for the duration of the period. This causes Michael to miss instruction and not complete work.

Both teachers report that Michael will often doodle with highlighters or pencils on his assignment when redirected to get on task. Mrs. Thomas was shocked since Michael does not display any of these behaviors during the times he is with her.

Use the “Student Goals and Intervention Template” to complete this assignment.

Part 1: Intervention Plan

Referring to the scenario provided, identify an operational definition of the target behavior, outline 3 aligning function of behavior goals, and an intervention plan for these goals.

Using the “Student Goals and Intervention Template,” identify and outline the following within Michael’s intervention plan:

  • Replacement behavior and specific steps to be implemented that will change the behavior based on information from teacher data 
  • Strategies to manage (reduce target behavior) situations and consequences based on information from observations.

Part 2: Crisis Plan

A crisis intervention plan are procedures to follow when reducing the target behavior is ineffective. The goal to maintain safety for all students and staff. Many schools and districts have crisis plans to follow with the goal of maintaining safety for all students and staff.

Create a descriptive crisis plan in a way that others can easily follow once it is determined that reducing the target behavior has been unsuccessful. Consider the following if the target behavior escalates:

  • How long should a staff member allow the target behavior to continue before implementing the crisis plan?
  • At what point should the student exhibiting the target behavior be removed from the classroom or when should other students be removed for safety? Where does the student or students go?
  • When should administration be involved?
  • At what point should local authorities or the school-based officer be contacted?
  • At what point should parents/guardians be contacted?
  • How will you know the crisis is over and full de-escalation has occurred? What happens then?

Part 3: Summary

After completing the intervention plan, include a 50-100 word summary describing the role Mrs. Thomas has in communicating Michael’s needs to and collaborating with his general education teachers .

Select a local species of interest to you as the topic for your inquiry lab. This should be a species well represented in iNaturalist (at least 50 sightings in the NY area). Once you have selected a species of animal, post the common and scientific name of your chosen species. (NO REPEATS – so check before you post). Now use iNaturalist to find out the following:

1. The specie’s biome/s (geographic habitat type), range local habitat type (use the map in iNaturalist)

2. What season/months do sightings peak? How might this relate to limiting factors for this species’ biotic potential? (click on the species name to see a green graph showing when its seen)

3. Which gender/sex is sighted more? (Click on the SEX tab) When are juveniles spotted? (click on the LIFE STAGE tab) Based on this, what type of survivorship curve do you think this species exhibits?

5. Identify one predator OR prey of this species found in the same areas (check iNaturalist)

Now generate a testable hypothesis about the sightings of your species in relation to a limiting factor/resource/prey/predator. eg. It is hypothesized that the red fox (Vulpes. vulpes) population will be highest in areas with a higher density of Eastern chipmunks (Tamias striatus).

This will tie in the work you have done in iNaturalist with the concept of Community Ecology. Now that you have selected a species and its predator/prey in iNaturalist, use Google Scholar to find two peer reviewed scientific research articles about the predator-prey relationship between these two species. Type scholar into Google and it takes you to Google Scholar. Now type in the scientific binomial of both species and a few search terms such as ”Predator-Prey Interaction” . Now read through the titles of the top articles that seem to include these terms and describe the impact of one on the other (its ok if other species are mentioned since they may be a surrogate for a similar comparison with your species). Its best to focus on the articles with a PDF icon somewhere. These can easily be downloaded for free (never pay!) From these, select two that seem most relevant to helping you understand the nature of the potential predator/prey interaction between these two species.

Post a brief summary of each article using only the Abstract of the articles. Summarize (in your own words!!!) two KEY findings of each study and briefly describe in two sentences how it could shed light on the predator prey relationship and/or population dynamics or distribution patterns of these two species. Begin each summary with the last name of the two lead authors and the year of publication… (eg Colon & Botton (2017) looked at…………and concluded………..).

Be sure to include a LINK to each article abstract so I can review that as well.

Scenario:

Danny is a 22-year-old college student, who has been brought into your office by his parents. Danny has agreed to let his parents be involved in his counseling. You first meet with Danny’s parents who explain to you that Danny has never been involved in counseling prior to this incident. They stated they felt his problems were not serious enough to bring him into counseling.

His mother reports that Danny can be the real life of the party and that most people find him very charismatic. She says that there was one incident in which Danny tried to harm himself due to a girlfriend cheating on him. She said that her husband felt that it was a pretty typical response for an adolescent. She said lately he has been staying up late playing video games and getting on the average of 2-3 hours of sleep per night. She said she worries about his lack of sleep but he doesn’t seem to show any signs that the lack of sleep is impacting his ability to function normally. He is currently getting all A’s in his college courses, but his recent incident has jeopardized his place at the university.

Danny comes into the office and explains to you that the whole incident is a big misunderstanding. He said to you that he doesn’t want to come to counseling because he is not crazy. He said his parents and the college administrators are requiring that he comes in for evaluation and commits to the recommendations of the counselor regarding if there is a need for therapy.

You ask him to tell you a little bit about the incident that caused him to come into your office. He explains to you that he really likes fast cars, but his parents don’t have a lot of money. He said he received a scholarship to a private university due to his outstanding grades. He said the only problem is that the other kids have a lot of money and can afford the items that he desperately wants but can’t afford. He said he saw another student leave his car running in the parking lot and went into the student center building. He said he was feeling like he was on top of the world and this was his golden opportunity to take that car for a spin. He said he had no plans for stealing the car, he was merely taking it for a test drive. When Danny came back to the building, campus security was interviewing the owner of the car. When he tried to explain to the other student and campus security he was taking it for a quick test drive, he was escorted into the building to talk to the administration. Since it’s a small private university, the student and administrators agreed not to press charges but he was placed on probation and had to commit to psych evaluation as well as following any recommendations made regarding therapy.

When you were speaking to Danny, you asked him if he was really trying to steal the car. He replies to you how dumb would he be to take the car back to the scene of the crime if he really had intended to steal the car. Then you ask him if he ever feels depressed. He says of course he does but he believes everyone has weeks or months where they just feel sad. You try to get him to engage further in the discussion, but he says he’s not here for you to diagnose him with depression.
