Gastrointestinal (GI) and hepatobiliary disorders affect the structure and function of the GI tract. Many of these disorders often have similar symptoms, such as abdominal pain, cramping, constipation, nausea, bloating, and fatigue. Since multiple disorders can be tied to the same symptoms, it is important for advanced practice nurses to carefully evaluate patients and prescribe a treatment that targets the cause rather than the symptom.  

Once the underlying cause is identified, an appropriate drug therapy plan can be recommended based on medical history and individual patient factors. In this Assignment, you examine a case study of a patient who presents with symptoms of a possible GI/hepatobiliary disorder, and you design an appropriate drug therapy plan.

Week 4: Gastrointestinal and Hepatobiliary Disorders

As an advanced practice nurse, you will likely encounter patients who will present with symptoms affecting the gastrointestinal (GI) tract. Of special note, is the consideration that most symptoms concerning the GI tract are non-specific and therefore, diagnosing diagnoses of the GI tract require thoughtful and careful investigation. Similarly, hepatobiliary disorders may also mirror many of the signs and symptoms that patients present when suffering from GI disorders.

How might you tease out the specific signs and symptoms between these potential disorders and body systems? What drug therapy plans will best address these disorders for your patients?

Discussion 2: Influence of Neuroscience on Cognitive Research

 

Scientists have engaged in study to determine whether or not the parts of the brain specialize in one task exclusively, performing an action repetitively like factory workers on a traditional assembly line. Some theorize that individual neural regions specialize in various tasks, at times even “contracting” one another for collaborative interactions (Kanwisher, 2010).

However the brain conducts its processing, the manner in which this occurs directly influences how humans appear on the “outside.” Thus, if human attitudes toward ethnically different groups correspond with “increased activity in the amygdala, dorsolateral prefrontal cortex, and anterior cingulate cortex,” (2008, p. 65) a deeper understanding of this activity may correlate with greater awareness of the mechanisms behind stereotyping and prejudice.

It is now possible to examine questions such as these using the arsenal of research methodologies available to cognitive research. Some examples of these methodologies include functional Magnetic Resonance Imaging (fMRI), Positron Emission Tomography (PET), Transcranial Magnetic Stimulation (TMS), and Magnetoencephalography (MEG).

For this Discussion, you identify a major impact of neuroscience on cognitive psychology. You then explain the impacts of neuroscience on social change. You also consider a related question you find interesting and describe effective research methods for investigating it. Locate five articles published in the last five years in peer-reviewed journals that document research involving questions and methodologies that you find interesting.

On-premise – refers to the traditional approach; a company purchases a license for an ERP system and install the system on its own server.

Tammy Potter, a new partner with the regional CPA firm of Tower & Tower, was recently appointed to the board of directors of a local civic organization. The chairman of the board of the civic organization is Lewis Edmond, who is also the owner of a real estate development firm, Tierra Corporation.

Potter was quite excited when Edmond indicated that his corporation needed an audit and he wished to discuss the matter with her. During the discussion, Potter was told that Tierra Corporation needed the audit to obtain a substantial amount of additional financing to acquire another company. Presently, Tierra Corporation is successful, profitable, and committed to growth. The audit fee for the engagement should be substantial.

Because Tierra Corporation appeared to be a good client prospect, Potter tentatively indicated that Tower & Tower wanted to do the work. Potter then mentioned that Tower & Tower’s quality control policies require an investigation of new clients and approval by the managing partner, Lee Tower.

Potter obtained the authorization of Edmond to make the necessary inquiries for the new client investigation. Edmond was found to be a highly respected member of the community. Also, Tierra Corporation was highly regarded by its banker and its attorney, and the Dun & Bradstreet report on the corporation reflected nothing negative.

As a final part of the investigation process, Potter contacted Edmond’s former tax accountant, Bill Turner. Potter was surprised to discover that Turner did not share the others’ high opinion of Edmond. Turner related that on an IRS audit 10 years ago, Edmond was questioned about the details of a large capital loss reported on the sale of a tract of land to a trust. Edmond told the IRS agent that he had lost all the supporting documentation for the transaction, and that he had no way of finding out the names of the principals of the trust. A search by an IRS auditor revealed that the land was recorded in the name of Edmond’s married daughter and that Edmond himself was listed as the trustee. The IRS disallowed the loss and Edmond was assessed a civil fraud penalty. Potter was concerned about these findings, but eventually concluded that Edmond had probably matured to a point where he would not engage in such activities.
