Scenario

As a security administrator for Always Fresh, you have been instructed to ensure that Windows authentication, networking, and data access are hardened. This will help to provide a high level of security.

The following are issues to be addressed through hardening techniques:

1. Previous attempts to protect user accounts have resulted in users writing long passwords down and placing them near their workstations. Users should not write down passwords or create passwords that attackers could easily guess, such as words founds in the dictionary.

2 Every user, regardless of role, must have at least one unique user account. A user who operates in multiple roles may have multiple unique user accounts. Users should use the account for its intended role only.

3 Anonymous users of the web server applications should only be able to access servers located in the demilitarized zone (DMZ). No anonymous web application users should be able to access any protected resources in the Always Fresh IT infrastructure.

4 To protect servers from attack, each server should authenticate connections based on the source computer and user.

Leadership in nursing is a key part in providing high quality healthcare to patients and in creating a conducive environment where staff are empowered. A nurse leader basically shows the other nurses the way and acts a bridge between them and the administrative leaders of the hospital. One of the responsibilities of a nurse leader is advocating for great patient care and needs of their unit nurses (Al-Dossary, 2017)by publicly supporting them. Another role is setting clear goals and accomplishing them successfully as well as building rapport with their followers. Application of creative thinking and problem solving skills should be part of day-to-day of a nurse leader. Other responsibilities include supervision of healthcare delivery, staffing and delegation of tasks to the nursing staff.

Education greatly contributes to nursing leadership skills; therefore, nursing education is crucial. A master’s education encompasses a course in leadership and care delivery which increases the effectiveness of leadership nursing as opposed to a newly registered nurse (Al-Dossary, 2017). For one to be an influential and successful nurse leader, strong interpersonal skills such as empathy and openness are very essential. This helps understanding the feelings of the staff and responding appropriately to them. Emotional intelligence is also significant since it leads to positive relationships between the nurse leaders and the staff (Hughes, 2017).This refers to the ability of one managing their emotions and those of others. These traits and more such as flexibility and integrity enhance good patient care, teamwork and promote a healthy working environment for nurses.

One of the most significant factors in empowering and encouraging nurses, who make up the vast majority of the healthcare workforce, to perform at the highest level of their licensure is nursing leadership. American Nurses Association (n.d) describes nursing leadership as “a nurse interested in excelling in a career path, a leader within a healthcare organization who represents the interests of the nursing profession, a seasoned nurse or healthcare administrator interested in refining skills to differentiate them from the competition or to advance to the next level of leadership.”

In my experience, one of the formal roles as a nurse leader is charge nurse for the respective department or unit. Although it’s normal to concentrate leadership efforts at the top of a hospital or health system, middle-management leaders (such as nurse practitioners) are critical to an institution’s progress. They have a direct influence on many front-line caregivers and healthcare staff. One of the formal roles of nurse leaders is the charge nurse for the respective department or unit. In our organization, to hold a position of charge nurse, one must have a bachelor’s degree and undergo charge nurse training classes held every month in terms of educational preparation. According to Felicia and Natalie (2020), the important responsibilities of charge nurse are;

  • Prioritizing patient safety protocols
  • Have vision and strategy to function the unit
  • Empower the entire healthcare team
  • Excel in problem-solving skills
  • Encourage interdependence and teamwork
  • Focus on quality patient care
  • Handle unit budget
  • Liaison between management and healthcare employees
  • Advocate for nurses as well as patients
  • Tackle staffing shortage issues

To be successful in this role, one must have qualities like good communication skills, competence, and confidence, willingness to learn new things, patience, good listening skills, and work together with the team. Encourage feedback and suggestions related to quality patient outcome, capacity to adapt in any situation, acknowledge work of team members, share credits of quality patient outcome, set ground rules, and maintain therapeutic relationships with patient and her team members. Charge nurses are suggestive of holding mentioned qualities to achieve planned patient outcomes and maintain a good relationship with interdisciplinary units and management. It is also needed to understand her staff and meet their needs to avoid conflicts and ensure patient safety. Ultimately nurses are continually confronted by new problems, practices, and resources as the healthcare landscape evolves. It will also make or break a nurse’s decision to stay or leave an institution whether they have a good nurse leader to help them handle these shifts.

During your practicum, determine what clinical problem or issue the organization is facing. Discuss two implications for nursing.

One of the issues i have witnessed at the organisation im working with is nursing shortage which leads to the nurses who are present to possibly face nursing burnout which could directly implicate and affect nursing through lack of or reduced patient safety and satisfaction. 

In the majority of studies studied, low health and moderate to high levels of burnout are linked to poor patient care results such as medical errors and lack of standard care ( Hall et al., 2016).Burnout is a “state of critical fatigue that occurs as a result of persistent stress and being over worked. The stress could is realated to work coupled with that os life, resulting in feelings of work-related exhaustion and a reduction of personal achievement. Nurse burnout causesnurses to experience feelings of dread about their jobs, mental and physical fatigue, sleep problems, and depression causing the nurses to become disengaged from their patients.

Two biggest implications will be increase in infections and low ratings with lack of patient satisfaction. The nurses will loose some focus at some point which may potenetially lead to patient disengagement. If patients do not trust and believe their nurses then a lot will go wrong with the health care system.

During your practicum, determine what clinical problem or issue the organization is facing. Discuss two implications for nursing.

Working in mental health hospital has been challenging as well as rewarding for me. After discussing practicum topics with my preceptor and understanding the clinical issues the organization is facing, I have decided to work on patient safety concerns for my capstone course.

Patient safety has been one of the major concerns working with patients with post-traumatic stress disorder. This program is newly started in our organization, treating patients with major depressive disorder, dissociative identity disorder, and post-traumatic stress disorder. Here patients are treated a little differently than other departments of the child, adolescent, adult, and eating disorder unit. Restrictions are not much as of others units. Here, patients are helped to work on their coping skills, internal work, and commitment to safety in the community. Safety concerns for self and others are considered a major priority in our trauma science program. According to World Health Organization (2021), public health issues are widespread: 450 million people worldwide suffer from mental or behavioral disorders, and almost one million people commit suicide each year. Depression, alcoholism, anxiety, and bipolar disorder are among the top ten causes of disease worldwide. In most countries, nurses are the largest group of professionals providing mental health care in both primary and specialist health services.

The two main nursing implications in mental health are building a professional nurse-patient relationship wherein the clear-cut boundaries are set for both patient and nurses, what can be discussed and what is not healthy for a therapeutic relationship. Because many times patients get so attached to particular nurses, based on how they deal with patients and become so dependent on these nurses to have them functioning their daily lives. Another essential nursing implication in inpatient care and especially in mental health nursing is effective communication, both verbal and nonverbal. For example, our patients have multiple triggers, so the right choice of words are crucial in developing a bond with these patients. They can read your body language very well, and they can very well manipulate things to work for them. Therefore, it is essential to establish a therapeutic nurse-patient relationship to help them in their treatment planning.

Until the 1980s, judges generally had wide discretion in handing down sentences. The system’s shift toward punishment brought with it changes in sentencing procedures. State legislatures began adopting sentencing guidelines that legally limited the judges’ discretion. In the past, some judges imposed sentences that were too lenient. Therefore, sentencing guidelines were developed in an effort to bring uniformity and rationality to sentencing and to curb what was perceived to be the excessive power of judges in matters of sentencing.

Currently, many states have their own sentencing guidelines, and the federal system has its own guidelines as well. Federal sentencing guidelines are regulations that lay down a uniform sentencing policy for convicted defendants in the US federal court system.
