You work for a large company that is hiring a social media director, a position the company has not had before. You know that some current employees may be qualified for the new position, so you want to encourage them to apply because they would already have knowledge about the company’s products, services, and culture. 

However, in the past few years, despite encouraging applications from current employees, new hires have been external candidates, and you know that some employees who applied for those positions have a little resentment about not getting hired. 

A full description of the position, including requirements and salary, is available on many job boards and the company’s website (www.companyname.com). Applications (résumé and cover letter) must be emailed to [email protected] by midnight of the last day of next month. Internal candidates must apply in the same way an external candidate would, by submitting their cover letter and résumé.

Write a MEMO announcing the new position. That memo will be distributed to all departments and posted publicly for everyone to see. Note, you should not describe the position in the memo; just refer employees to the complete job description provided on the company’s website. The purpose of the memo is to encourage current employees to apply for the job (and refer them to the detailed job description on the website).  

Remember to use all principles of writing studied so far in the course (Six Cs of Communication, positive emphasis, “you” approach, message formatting and organization, etc.). If you are writing more than 2-3 short paragraphs, you are writing too much!

Focused SOAP Note and Patient Case Presentation

For this Assignment, you will document information about a patient that you examined during the last three weeks, using the Focused SOAP Note Template provided. You will then use this note to develop and record a case presentation for this patient. 

To Prepare

  • Review this week’s Learning Resources and consider the insights they provide. Also review the Kaltura Media Uploader resource in the left-hand navigation of the classroom for help creating your self-recorded Kaltura video.
  • Select a patient of any age (either a child or an adult) that you examined during the last 3 weeks.
  • Create a Focused SOAP Note on this patient using the template provided in the Learning Resources. There is also a completed Focused SOAP Note Exemplar provided to serve as a guide to assignment expectations.
    Please Note:

    • All SOAP notes must be signed, and each page must be initialed by your Preceptor. Note: Electronic signatures are not accepted.
    • When you submit your note, you should include the complete focused SOAP note as a Word document and PDF/images of each page that is initialed and signed by your Preceptor.
    • You must submit your SOAP note using SafeAssign. Note: If both files are not received by the due date, faculty will deduct points per the Walden Grading Policy.
  • Then, based on your SOAP note of this patient, develop a video case study presentation. Take time to practice your presentation before you record. 
  • Include at least five scholarly resources to support your assessment, diagnosis, and treatment planning.

The Assignment

Record yourself presenting the complex case study for your clinical patient. In your presentation:

  • Dress professionally with a lab coat and present yourself in a professional manner.
  • Display your photo ID at the start of the video when you introduce yourself.
  • Ensure that you do not include any information that violates the principles of HIPAA (i.e., don’t use the patient’s name or any other identifying information).
  • Present the full complex case study. Include chief complaint; history of present illness; any pertinent past psychiatric, substance use, medical, social, family history; most recent mental status exam; current psychiatric diagnosis including differentials that were ruled out; and plan for treatment and management.
  • Report normal diagnostic results as the name of the test and “normal” (rather than specific value). Abnormal results should be reported as a specific value.
  • Be succinct in your presentation, and do not exceed 8 minutes. Specifically address the following for the patient, using your SOAP note as a guide:
    • Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
    • Objective: What observations did you make during the psychiatric assessment? 
    • Assessment: Discuss their mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses and why you chose them. List them from highest priority to lowest priority. What was your primary diagnosis and why? Describe how your primary diagnosis aligns with DSM-5 diagnostic criteria and supported by the patient’s symptoms.
    • Plan: What was your plan for psychotherapy? What was your plan for treatment and management, including alternative therapies? Include pharmacologic and nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan. Also be sure to include at least one health promotion activity and one patient education strategy.
    • Reflection notes: What would you do differently with this patient if you could conduct the session again? If you are able to follow up with your patient, explain whether these interventions were successful and why or why not. If you were not able to conduct a follow up, discuss what your next intervention would be.

Three instructional plan templates constructed by a variety of leaders in education provide solid examples of what quality instructional plans should include. The work of Madeline Hunter dates the furthest back and is still used today, primarily in the elementary setting. Grant Wiggins and Jay McTighe provide a more modern approach to curriculum and lesson design with their model of Understanding by Design (UbD). Others, as modeled by the New York State Educational Department, work closely to align their instructional plans with the Common Core State Standards.

Analyze each instructional plan and structure a Word document, essay-style as such:

  1. Introduction: Introduce the essential elements, purpose, and value of creating and following a high-quality instructional plan. Include a thesis stating your intent to highlight key elements of each respective plan as well as your intent to identify what you find to be the most effective plan while justifying your reasoning.
  2. Body: Discuss the following for EACH instructional plan design. (Do not list—this is paragraph format without headings/subheadings.)

Every case is different. It also depends who you ask. The more you try to rationalize on how a physician is liable for the sake of pointing fingers the more touch you lose of the issue at large. We are living in an era where medicine continues evolving on a day to day basis. Do I believe physicians should be held liable when accountable? Absolutely. If an event or early detection was unforeseeable, I do not believe a physician should be at fault. As previously stated, every case is different.

In order to properly diagnosed a patient, a physician must follow a protocol to make that diagnosis. The patient can have all the signs and symptoms of a particular condition, but then again differential diagnosis’ look very similar to one another hence the term “differential diagnosis.” A physician can never be 100% sure until appropriate diagnostics are completed. Nevertheless, there are patients that are dishonest with their physician. A 31 year old man comes to your office for an annual check-up. You take their vitals and discover the patient is hypertensive. Upon discovery, you proceed with the patients full history and current information. The physician then concludes that this patient has no familial history of cardiovascular diseases, maintains a proper diet, maintains a healthy fitness lifestyle, etc. You do not see why this patient is hypertensive. The physician then proceeds with prescribing a beta blocker to this patient. A week later the patient dies due to a hypertensive crisis. What happened? The patient never disclosed their cocaine abuse to their physician and the physician never ordered the labs to test for cocaine and other drug abuse. Should the physician be held liable? I would think so. A differential diagnosis must be found and all other possible conditions ruled out. Although the patients never fully disclososed their lifestyle, the physician should have order the appropriate test given the patient’s history.

In the case referenced in the textbook, the loss of chance policy would be very beneficial to both parties. It would ensure that the physician has done everything in their power to find a solution to the cause. Essentially, the physician must dig deeper and exclude every differential diagnosis possible until a solution is found.
