To Prepare:

  • Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry. Keep in mind that the clinical issue you identify for your research will stay the same for the entire course. 
  • Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least four different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
  • Review the results of your peer-reviewed research and reflect on the process of using an unfiltered database to search for peer-reviewed research.
  • Reflect on the types of research methodologies contained in the four relevant peer-reviewed articles you selected.

Steps:

Third, to successfully synthesize the five sources, begin by grouping sources by topic and looking for connections across the readings. For example, look for overlapping ideas, where sources agree with each other, and divergences. Any unanswered questions or identifiable limitations of the readings? Fourth, begin to make an argument based on the readings and what they tell us about the broader topic (the Module’s focus is the broader topic). Fifth, after you understand what the sources are arguing, you want to construct your body paragraphs in a way that acknowledges different sources and highlights where you can draw new conclusions. HIMS Organization

Scenario

You are the senior manager of a large health care organization. The senior management team must select a HIMS for the organization that will encompass several clinical and administrative departments. You will need to create and deliver a PowerPoint presentation that will persuade your CEO to purchase your chosen HIMS. Your presentation also must describe key aspects of HIMS focusing on the electronic medical record, such as implementation, interoperability, productivity, and support challenges. Use the work you did and the feedback you received from the Week 9 discussion when creating your presentation.

Instructions

Create a 10–12 slide audio PowerPoint presentation with speaker notes (maximum of 20 minutes) in which you:

  1. Justify to the CIO the need to integrate all clinical and administrative departments into a HIMS in your health care organization.
  2. Select the most significant and current HIE, EHR, HIPAA, and HITECH regulations in your state. Next, determine two ways these regulations could impact the integration of HIMS in your health care organization. Include three potential solutions to address these regulation challenges.
  3. Propose three privacy and security measures to help your organization’s health care providers avoid security breaches and data loss, while better allowing them to concentrate on patient care. Next, develop an action plan to protect patient information that complies with HIE, EHR, PHI, and HIPAA legal requirements.
  4. Suggest three key actions to monitor privacy and security violations that may occur after the HIMS implementation in your health care organization. Summarize in one slide your key findings. Gastrointestinal Function:
R.H. is a 74-year-old black woman, who presents to the family practice clinic for a scheduled appointment. She complains of feeling bloated and constipated for the past month, some-times going an entire week with only one bowel movement. Until this episode, she has been very regular all of her life, having a bowel movement every day or every other day. She reports straining most of the time and it often takes her 10 minutes at a minimum to initiate a bowel movement. Stools have been extremely hard. She denies pain during straining. A recent colonoscopy was negative for tumors or other lesions. She has not yet taken any medications to provide relief for her constipation. Furthermore, she reports frequent heartburn (3–4 times each week), most often occur-ring soon after retiring to bed. She uses three pillows to keep herself in a more upright position during sleep. On a friend’s advice, she purchased a package of over-the-counter aluminum hydroxide tablets to help relieve the heartburn. She has had some improvement since she began taking the medicine. She reports using naproxen as needed for arthritic pain her hands and knees. She states that her hands and knees are extremely stiff when she rises in the morning. Because her arthritis has been getting worse, she has stopped taking her daily walks and now gets very little exercise.

Case Study Questions

  1. In your own words define constipation and name the risk factors that might lead to develop constipation. List recommendations you would give to a patient who is suffering from constipation. You might use a previous experience you might have.
  2. Based on the clinical manifestations on R.H. case study, name and explain signs and symptoms presented that are compatible with the constipation diagnosis. Complement your list with signs and symptoms not present on the case study.
  3. Sometimes as an associate diagnosis and a complication, patients with constipation could have anemia. Would you consider that possibility based on the information provided on the case study?

Endocrine Function:
C.B. is a significantly overweight, 48-year-old woman from the Winnebago Indian tribe who had high blood sugar and cholesterol levels three years ago but did not follow up with a clinical diagnostic work-up. She had participated in the state’s annual health screening program and noticed that her fasting blood sugar was 141 and her cholesterol was 225. However, she felt “perfectly fine at the time” and could not afford any more medications. Except for a number of “female infections,” she has felt fine until recently. Today, she presents to the Indian Hospital general practitioner complaining that her left foot has been weak and numb for nearly three weeks and that the foot is difficult to flex. She denies any other weakness or numbness at this time. However, she reports that she has been very thirsty lately and gets up more often at night to urinate. She has attributed these symptoms to the extremely warm weather and drinking more water to keep hydrated. She has gained a total of 65 pounds since her last pregnancy 14 years ago, 15 pounds in the last 6 months alone.

  1. In which race and ethnic groups is DM more prevalent? Based on C.B. clinical manifestations, please compile the signs and symptoms that she is exhibiting that are compatible with the Diabetes Mellitus Type 2 diagnosis.

Simchi-Levi et al.(2008) noted that consumer product makers such as Procter & Gamble pioneered rebates in the 1970 as a nitty way to advertise small discounts without actually marking the price down,  The reason why rebates are popular is because while it pushes consumers to purchase products many rebates are not claimed – about 40% of rebates are unclaimed totaling more than $2 billion extra revenue to retailers. Simchi-Levi et al.(2008) noted that what the rebates do is get consumers to focus on the discounted price of a product, then buy it at full price(p. 402).

Using rebate instead of price reduction can be viewed as an example of customized pricing because some people may not spend time on redeeming rebates. It is an example of customized pricing because it factors both the customers who really need rebate and those who are looking for the quality product without worrying about price. Customized Pricing is a method of determining the price for a good or service based on the perceived expectations of customers. Obermiller C, Arnesen D & Cohen M (2012) noted that with customized pricing, identical products are delivered, regardless of time or situation, to different consumers at different prices. Customized pricing can also be dynamic pricing, if each consumer is presented with a price that is determined by an estimate of that individual demand for a given product, in a specific situation and time. Many markets feature dynamic pricing that is not individualized.

One of the reasons why manufacturers might offer rebates rather than decrease wholesale prices even if all rebates were redeemed because while rebates attracts more consumers to store, a reduction in wholesale prices might not.

Also applying rebates instead of decrease in wholesale prices have an impact on profitability and costs. Demirag, et al. (2009) noted that manufacturers use promotions directed to the end customers and/or retailers in their distribution channels to increase sales and market share. The consumers pay the full price, and this increases the company’s cash flow as well as sales volumes and boost stock price. When the rebates are redeemed, it is recorded as an expense. Overall, rebates increases profitability even if they were redeemed because of increased sales which wouldn’t have been achieved with decrease in wholesale prices.

4-Each alpha level is dependent on the circumstance that surrounds a particular study. The significance level(alpha) is the probability of committing a type 1 error. A type 1 error is committed when the researcher falsely rejects the null hypothesis. A significance level of 0.05 is the standard situation, most especially in the field science.

There are some experiments where you would most likely want to lower the type 1 error rate such as experiment that affects human health, like drug research or studies of psychological treatment. For some experiments, if the consequence of applying null hypothesis is extremely serious, for instance, if null hypothesis applies, there may be death, or serious injury, then you want to try your best to avoid the type I error. That means you must avoid the situation that null hypothesis is true but you reject it. As the significance level is the probability, you will make the type 1 error. So, for such experiments with serious results, we want to make the level smaller than standard situation. So, for such experiments, if you can’t tolerate a 5% chance of being wrong, use a lower significance level, 0.01 for example. 0.01 is common if there’s a possibility of death or serious disease or injury.

If the consequences of being wrong are especially minor such as political research or animal migration studies. you might use a higher significance level, such as 0.1, but this is rare in practice. That is, it may be common that we make the significance level much smaller than 0.05, but we rarely make the level larger than 0.05.

5-The alpha is the level of statistical significance. It can be any number between 0-1. 0.10, 0.05 and 0.01 are most commonly used. A situation where we would want to accept a higher alpha level is with medical testing. We would much rather have false positive test results that would lead to additional testing, even though it is going to give our patients an insane amount of anxiety. It is better than a false negative where no further testing or treatment would be indicated, and the patient would go untreated.

6-Not all results of hypothesis tests are equal. A hypothesis test or test of statistical significance typically has a level of significance attached to it. This level of significance is a number that is typically denoted with eh Greek letter alpha Many journals throughout different disciplines define that statistically significant results are those for which is equal to 0.05 or 5%.

The number represented by  is a probability, so it can take a value of any nonnegative real number less than one. Although in theory any number between 0 and 1 can be used for , when it comes to statistical practices this is not the case. Of all levels of significance, the values of 0.10, 0.05, and 0.01 are the most commonly used .

In medical screening for a disease, consider the possibilities of a test that falsely tests positive for a disease with one that falsely tests negative for a disease; a false positive will result in anxiety for our patient but will lead to other tests that will determine that verdict of our test was indeed incorrect; a false negative will give our patient the incorrect assumption that he does not have a disease when he in fact does. The result is that the disease will not be treated; given the choice, scientists would rather have conditions that result in a false positive than a false negative.
