Introduction: Communication Change Challenge (CCC) Part 8
Part 8 is where you will evaluate the impact of the project as it relates to interpersonal communication and the course objectives (COs). It is important here that you clearly demonstrate your understanding of many of the course concepts through application to your course project.

Instructions
Remember that your patterns for communicating interpersonally have been developed over many years. Therefore, you should be neither surprised nor discouraged to find that changing your interpersonal communication behavior takes time. In order for you to take this project to its fullest potential, you may need to continue to modify, implement, and reinforce these new behaviors for far longer than the duration of the project. It is possible, however, to modify the way you communicate, and because the payoff is improvement to our communication and relationship outcomes, changing interpersonal communication is worth the effort. You have now implemented your target behaviors for about two weeks, write a final evaluation addressing the following:

This week, you examined more closely learning goals, outcomes, and objectives and how they factor into the design process. Think of a road trip. Generally, you have a map to guide you on your journey. Without it, you run the risk of getting lost. Unless getting lost is your objective, that map will help you stick to the best path to get you where you need to be. If you don’t know where you are going, how will you know when you get there? This is what goals and objectives, as a part of the overall ID process, do for instruction.

The HITECH Act was incorporated into ARRA to promote the adoption and meaningful use of health information technology. Case I: The Blue Wall

Many patients and regulators have accused the hospital of neglecting its organizational responsibilities to respect patient rights. The hospital has established a so-called blue wall to withhold information and protect its employees. The administration and the ethics committee overseeing these ethical issues were accused of cover-up and making decisions that endangered vulnerable people. In most cases, the hospital has failed to meet its responsibilities to patients and to comply with regulations. Some of the violations are the following:

  • Employees have exposed patient information to unauthorized people.
  • Nurses have made unilateral decisions and ignored informed consent mandates.
  • Administrators have covered up instances of medication errors and failed to meet regulatory compliance regarding the handling, storage, and retention of medical records.
  • Visitors have found sensitive patient information in files left in hallways and on laptops left in patients’ rooms. Mobile devices containing patient information that doctors have claimed were missing have been found lying around in public areas.

In addition to establishing universal coverage and shared responsibility, the Affordable Care Act sets federal standards for health insurers offering products in both the individual and small-group markets, as well as employer-sponsored health benefit plans, Rosenbaum S. (2011).

The law has 3 primary goals, according the Healthcare.gov:

1. Make affordable health insurance available to more people. The law provides consumers with subsidies (“premium tax credits”) that lower costs for households with incomes between 100% and 400% of the federal poverty level (FPL). Note: If your income is above 400% FPL, you may still qualify for the premium tax credit in 2021.
2. Expand the Medicaid program to cover all adults with income below 138% of the FPL. (Not all states have expanded their Medicaid programs.)
3. Support innovative medical care delivery methods designed to lower the costs of health care generally.

The Affordable Care Act was designed to reduce the cost of health insurance coverage for people who qualify. The law includes premium tax credits and cost-sharing reductions to help lower expenses for lower-income individuals and families. Premium tax credits lower your health insurance bill each month. It was developed to make health insurance more affordable, to initiate consumer protection, and to increase the number of people covered by health insurance.
Nurses can support the health of families and communities by serving as an important resource in providing information on how to access the health insurance coverage that the Affordable Care Act (ACA) offers.

Management of heart disease is a major focus in health care today. Treatment modalities for the various complications that occur because of heart disease are some of the more costly treatments in all of health care. One specific procedure, transcatheter aortic valve replacement (TAVR), is an effective option for CAD patients needing valve replacement and who are often too high of a surgical risk for traditional or standard surgery. According to Faroux et al., (2019), about one half of TAVR candidates have coronary artery disease (CAD). However, TAVRs are being performed in inconsistent facilities with varying levels of experience (Faroux et al, 2019). Because of this, it is essential that healthcare providers have a better understanding of the quality-of-life and cost-effectiveness of TAVRs as compared to other treatment modalities. Gialama et al., (2018), explain that valvular heart disease (VHD) is estimated to be at 2.5% currently in industrialized countries; and is markedly increased after 65 years of age. Therefore, the prevalence, and proposed treatments, need further exploration to ensure the most appropriate treatments is provided in the most cost-effective manner.

To determine cost-effectiveness, a systematic review by Gialama et al., (2018) summarized 24 studies published between 2012 and 2017, which focused on the economic evaluation of transcatheter aortic valve implementation and transcatheter mitral valve repair versus standard surgical procedures. Although the conclusion of this review indicates there is cost-effectiveness, the limitations of this study, and need for further research make the outcomes questionable (Gialama et al., 2018).

  • Does the research design answer the research question? Explain your rationale.

The short answer is yes; however, more research is needed. Another point to consider is not only the cost-effectiveness of the procedures themselves, but the long-term impact. Will patients need more healthcare, or less? Determining cost-effectiveness needs to include more than just the costs of the procedures and associated costs; but the costs of needed care “down the road”. If the procedure is a “one and done” then, it is clearer, but if the procedure is only a band-aid for now, then is it really cost-effective? Additionally, limitations of the review, discussed below, continue to support the question of valid proof of cost effectiveness (Gialama et al., 2018).

  • Were the study sample participants representative? Why or why not?

No. The studies included in this review were limited (further discussed below). The criteria for this review was very specific; but may have omitted studies done in other areas, and published in other languages, which may have provided much-needed enhanced information. Exclusions included: studies in other languages, abstracts, editorials, letters, case reports, conference presentations, expert opinions, reviews, all of which could provide more data to substantiate the cost-effectiveness, or not (Gialama et al., 2018).

  • Compare and contrast the study limitations in this study.

Limitations in this review are primarily related to the exclusions. The review included 24 studies, all in the English language, and many needing further research. Based upon this information, the limitations of the study are largely excluding patient data which would be helpful in fully determining the cost-effectiveness for both the short-term and long-term effect.  

  • Based on this evidence summary, would you consider this systematic review as support for your selected practice problem? Explain your rationale.

This systematic review provides a foundation for understanding of the cost-effectiveness of the TAV procedures, however, until more is known about studies conducted in more diverse settings, and a more streamlined process for evaluation in comparable institutions and experience, the results from this review are not conclusive enough to make a definitive determination of the overall cost-effectiveness of this treatment modality.

Defining Suppliers & Channel Partners

 This week we will again examine a real-life situation.  The event under examination will be the engineering failures related to the Boeing 737MAX airframe.Our examination will have three steps as follows:

  1. We will review the report provided in pdf format immediately below.
  2. We will undertake to discern the relevant stakeholders, relevant issues, and salient facts.
  3. We will use our knowledge to identify where leadership failed and suggest methodologies for avoiding a repetition of events and results.

The below report provides both technical and non-technical data outlining the causes of the 737MAX design failure.  You may conduct research so as to have a better understanding of this case.  Your presentation, however, may only use that information presented in the report.  Based on this report alone, your assignment is to create a PowerPoint presentation as follows:

  1. Slide 1 – create an opening PowerPoint slide which presents your name, the date and a title for your presentation. The format of this slide is at your discretion. 
  2. Slide 2 – identify the individual or organization that you believe was most at fault for this design failure.  The format of this slide is at your discretion.
  3. Slide 3 & 4 – focusing solely on the individual or organization you identified in Step 2 (immediately  above), identify the primary cause which contributed to their defective design decisions.  This is your identified issue vis-a-vis the party identified in Step 1 (immediately above).  Frame this identified causality in the form of both a question (slide 3) and as a sentence (slide 4).  On each slide report if this causality is acute or chronic, an issue or a dilemma.  The format of the slide is at your discretion.
  4. Slide 5 – identify the facts which are relevant to the causality your identified in Step 2 (immediately above).  You are limited to four (4) “simple sentences” or bullets.  Investigate the meaning of “simple sentence” before undertaking this part of the exercise.  Please present these facts on a PowerPoint slide in the format you select.
