physical assessment

Module 03 Content

  1. You will perform a history of a musculoskeletal problem that your instructor has provided you or one that you have experienced and perform an assessment of the musculoskeletal system. You will document your subjective and objective findings, identify actual or potential risks, and submit this in a Word document to the drop box provided. Your subjective portion of the documentation should briefly describe your “client”. For example, “This client is a 23-year-old white female complaining of a painful, swollen ankle. States that she stepped ‘funny’ off a step two days ago and thinks she heard a ripping sound. She takes no medications and has no allergies. The client reports pain as 5/10 with sharp twinges when trying to walk, resting and ice decreases pain to 2/10 aching. Pain is primarily in the outer aspect of ankle and foot. Has no prior injury to this area. No significant past medical history.” In terms of your objective findings, remember to only record what you have assessed. Do not make a diagnosis or state the cause of a finding. You are not coming to any conclusions within your documentation. When your documentation is complete, you will note any findings that were abnormal.

For this Check-Up, you are acting as a counselor. You will find in your career that often you will need to provide more than medical advice: sometimes you have to provide emotional support as well. In this case, you need to counsel your patient, about what he suspects he is experiencing.

Henok is a 55-year-old male. He was born in Addis Ababa, Ethiopia but has lived in the American South for the last 30 years. He has had a successful life as an industrial engineer, and has made a very lucrative living. Henok got married when he was 45 years old and had twin boys when he was 47. He loves his wife and twin boys, but he is starting to feel restless and anxious and isn’t sure why. He signed up for a skydiving trip with his friends two weeks ago and loved it. Now he wants to buy a motorcycle. He has come to you because he is experiencing chest pains. His EKG is normal, and all his bloodwork is normal. Everything points to him being physically healthy.

You ask a few questions and find out the following information:

  • He hasn’t been sleeping well.
  • He thinks he is a failure.
  • He recently had his black hair bleached blonde,
  • He bought a beach house and hasn’t told his wife yet.
  • He got the phone number of a female coworker to meet for drinks, but he felt guilty and threw it in the trashcan right away.
  • He has found himself missing his late father.

Your job:

  1. Tell Henok what you think is going on (1-2 paragraphs)
  2. Tell Henok how you think he can best overcome these feelings (2-4 paragraphs)
  3. Make a referral for Henok (1 paragraph)
  4. Provide some encouragement for Henok (this is your conclusion)

Pathophysiology Project

Pathophysiology is the study of how a disease progresses in the body. As you can imagine, there are many disorders and diseases that can occur. The purpose of this individual project is to be introduced to these many different diseases or disorders via research. In this project, you will research and report on a specific disease or disorder. The project will span the entire duration of the course and have deliverables due in many modules, with the complete project–a 4- to 5-minute presentation with a written component–due in Module 7. Over the last few modules, you have progressed through the research of your disease. It is now time to put your work into a viewable presentation. In this module, you will turn in a rough draft of your presentation. In the next module, you will create a video of your presentation.

The disorder I have selected is Marfan Syndrome.

Marfan syndrome is an inherited disorder that affects connective tissue — the fibers that support and anchor your organs and other structures in your body. Marfan syndrome most commonly affects the heart, eyes, blood vessels, and skeleton. People with Marfan syndrome are usually tall and thin with unusually long arms, legs, fingers, and toes. The damage caused by Marfan syndrome can be mild or severe. If your aorta is affected, the condition can become life-threatening. Treatment usually includes medications to keep your blood pressure low to reduce the strain on your aorta. Regular monitoring to check for damage progression is vital. Many people with Marfan syndrome eventually require preventive surgery to repair the aorta.

Part 1:

  1. Find three additional citations that could be used for your presentation topic. Include the citations in full APA 7th edition format in the same Microsoft Word document you used in Module 2.
  2. Annotate each of these citations with a separate 5-sentence paragraph, including the following information:
  • Description of the source and author: Are they reliable and valid? [1 sentence]
  • Description of findings that may be important to your project. [2–3 sentences]
  • Include the reason why you chose the source. How will it support your project? [1 sentence]
  • In one short paragraph, discuss the formatting for the project and reasons for this format. This will be your project format proposal. You will be required to present your project in Module 6 with a 4-5 minute presentation; however, the format of the project is up to you. Use the Pathophysiology Project Rubric and Guide to help you figure out which format you would like to use. Here are some options:
  1. PowerPoint presentation

Trait Behavior and Influence

One of the first systematic attempts to study leadership is the Trait Approach.  Trait behavior is positively correlated to ‘influence.’  According to Rendall (2006), Influence with others are the most basic element of leadership.  The Trait Approach focuses on identifying innate qualities and characteristics possessed by great social, political and military leaders. The Trait Approach also includes traits to either ‘possess’ or ‘cultivate’ if one seeks to be perceived by others as a leader (i.e. Intelligence, Self-Confidence or Determination).

Former NBA player, Michael Jordan is known as the GOAT (greatest of all times).  According to Michael Jordan,

One day, you might look up and see me playing the game at 50. Don’t laugh. Never say never, because limits, like fears, are often just an illusion. Some people want it to happen, some wish it would happen, others make it happen.I can accept failure, but I can’t accept not trying.Click the link below and view Michael Jordan’s commercial, “Maybe,” on YouTube.

  1.  What is the main message of the commercial?
  2. What do the supporting images tell viewers about Jordan’s extraordinary career?  Recall specific examples.
  3. What traits of Jordan’s are suggested by this commercial?
  4. What traits were demonstrated to positively influence the follower?
  5. In way ways might thee traits of an excellent leader be demotivating to a follower?

Organizational Change and Workforce Management

Saudi Hospital System (SHS, a fictitious organization) has been facing many serious issues including leadership problems, staffing deficiencies, and financial hardship. Challenges include:

  • High staff turnover, which doubled to 20% in the past two years, is expected to rise.
  • SHS is in the process of implementing complex and expensive multi-year healthcare information systems such as Electronic Health Record and other critical-need clinical information systems.
  • SHS is facing significant competition from other local healthcare facilities.

Last year, a new Chief Executive Officer (CEO) was hired. He identified the current organizational culture as apathetic, leaderless, and resistant to change. He also noted that the staff was under stress, resisting teamwork, accepting mediocrity, and compromising patient safety values and industry best practices. The CEO noted that critical human resources functions were broken and that financial performance was suffering. He attributed this to a variety of process issues as well as to the lack of focus on the core business of patient care.

In response, he decided to implement the following strategies:

  • change the organizational culture
  • improve human resources and workforce practices and outcomes
  • ensure the organization’s solvency and financial viability in the long run
  • cut costs across the board, including implementing a labor-reduction strategy (salary and benefits), with an aggressive timeline to turn around the financial bottom line.

Within a few months after these strategies were implemented, the SHS started to show less of a financial loss and seemed to stabilize financially. The morale of the staff, however, took a significant hit in the wrong direction. Staff turnover increased as the sense of job security decreased and as an increasing number of valued and critical staff in all areas left. Soon, SHS experienced an increase in patient complaints and lowered customer service, with signs of impact on clinical patient outcomes.

Now the CEO wants to create a dynamic long-term organization and human resources strategic plan to put SHS on the right track. To formulate and implement this plan, the CEO hired a well-known consultant who is tasked with performing a system-wide organization and human resources assessment.

Within the scope of this assessment is the development and formulation of a multilevel strategic program. This new program is to be implemented system-wide with the goal of helping SHS leaders to understand the link among finances, employee morale, human resources functions, and patient satisfaction. The end goal of this assessment is to create a culture of accountability; to shift the culture to one of services after two years; to attract, hire, and retain quality staff; and to implement best practices in workforce management.

Assume that you are the consultant whom the CEO hired to do this assessment project. Your main task is to identify the key components involved in changing the organizational culture and to identify the steps needed to generate sustainable change. Here are the deliverables that the CEO wants to see from you:

  1. Develop a comprehensive organization and human resources strategic plan. The plan has two major sections: Organization Development and Human Resources.  The CEO expects you to create your plan using benchmarking and by integrating human resources and organization development best practices from other healthcare organizations, as well as from other industries.
  2. The Organization Development section will address the following items:
    • Workplace communication
    • Motivation
    • Conflict Management.
  3. The Human Resources section will address the following items:
    • Organizational and human resources systems that need to be in place to effect positive organizational outcomes
    • Performance management system
    • Process of training and development and the contribution to the organization’s bottom line
    • Key action steps required to develop the plan.

Your well-written paper should meet the following requirements:

  • Be 6-8 pages in length, not including the required cover and reference pages.
  • Formatted according to  APA writing guidelines.
  • Provide support for your statements with in-text citations from a minimum of six scholarly articles from peer-reviewed journal articles.
