A small group of nurses, employed at a large community hospital, were unhappy about their work environment and would meet daily during lunch to discuss the situation. There had been a recent change in the hospital’s senior management, which caused a high level of uncertainty and anxiety among the nursing staff. The nurses felt overworked as a result of the industry’s current nursing shortage. Their wages and benefits had been stagnant, with no salary market adjustments for the past two years. The nurses saw the situation as management requiring them to do more work with fewer resources, with no appreciation or recognition of their efforts. Whenever the nurses approached management with their concerns, they perceived them as falling on deaf ears since no changes were made.

Feeling like they had no other choice, the nurses contacted a labor union. The labor union began an organizing effort in the hospital shortly thereafter, holding an aggressive campaign over a six-week period. There was tremendous peer pressure, as some of the well-respected nursing staff became active leaders for unionization, although they were not part of the initial group of nurses who had contacted the union. The election was held, and the union was voted in by two-thirds of the nursing staff. In the weeks that followed, the original group of nurses remarked that they were surprised by the union’s victory; they had only wanted to scare management into making changes to their work environment.

For this assignment, you will develop a proposal for a training program on facilitating change within an organization.

Scenario:

You have been hired as a training consultant, and you are responsible to develop a training program for managers tasked with facilitating change within an organization.

Tasks:

Identify an organization in which you have been involved. Based on the material covered in this course as well as the knowledge gleaned from your work experience, develop a proposal that addresses the following:

  • Describe the organization for which you are developing the training proposal. In your description, include a discussion of the industry (including current industry trends) and the organization’s customers.
  • Identify the organizational characteristics that will influence decisions for how change initiatives will be planned and implemented.
  • Provide recommendations for developing effective change leaders and followers. Your recommendations should address the need for healthy change facilitation as well as the demands placed on a change facilitator.

 The patient is an 11-year-old girl who has been complaining of intermittent right lower quadrant pain and diarrhea for the past year. She is small for her age. Her physical examination indicates some mild right lower quadrant tenderness and fullness.

Hematocrit (Hct), 28% (normal: 31%-43%) 

Vitamin B12 level, 68 pg/mL (normal: 100-700 pg/mL)

D-Xylose absorption, 60 min: 8 mg/dL (normal: >15-20 mg/dL) 120 min: 6 mg/dL (normal: >20 mg/dL)

 Lactose tolerance, No change in glucose level (normal: >20 mg/dL rise in glucose) 

Small bowel series, Constriction of multiple segments of the small intestine 

Diagnostic Analysis 

The child’s small bowel series is compatible with Crohn disease of the small intestine. Intestinal absorption is diminished, as indicated by the abnormal D-xylose and lactose tolerance tests. Absorption is so bad that she cannot absorb vitamin B12. As a result, she has vitamin B12 deficiency anemia. She was placed on an aggressive immunosuppressive regimen, and her condition improved significantly. Unfortunately, 2 years later she experienced unremitting obstructive symptoms and required surgery. One year after surgery, her gastrointestinal function was normal, and her anemia had resolved. Her growth status matched her age group. Her absorption tests were normal, as were her B12 levels. Her immunosuppressive drugs were discontinued, and she is doing well.

  CASE STUDY 5: URINARY OBSTRUCTION

 The 57-year-old patient noted urinary hesitancy and a decrease in the force of his urinary stream for several months. Both had progressively become worse. His physical examination was essentially negative except for an enlarged prostate, which was bulky and soft.

Diagnostic Analysis 

Because of the patient’s symptoms, bladder outlet obstruction was highly suspected. Physical examination indicated an enlarged prostate. IVP studies corroborated that finding. The reduced urine flow rate indicated an obstruction distal to the urinary bladder. Because the patient was found to have a normal total voided volume, one could not say that the reduced flow rate was the result of an inadequately distended bladder. Rather, the bladder was appropriately distended, yet the flow rate was decreased. This indicated outlet obstruction. The cystogram indicated that the bladder was capable of mounting an effective pressure and was not an atonic bladder compatible with neurologic disease. The tonus limb again indicated the bladder was able to contract. The peak bladder pressure of 50 cm H2O was normal, again indicating appropriate muscular function of the bladder. Based on these studies, the patient was diagnosed with a urinary outlet obstruction. The PAP and PSA indicated benign prostatic hypertrophy (BPH). The ultrasound supported that diagnosis. Cystoscopy documented that finding, and the patient was appropriately treated by transurethral resection of the prostate (TURP). This patient did well postoperatively and had no major problems.

Jailhouse Informants

Prologue:

Due to Badpenny’s financial situation, she is unable to post bail because her bond is set too high.  The charges Badpenny faces include possession of methamphetamine (felony), possession of drug paraphernalia (misdemeanor), and official misconduct – using her position as a dispatcher to access confidential information for personal gain (misdemeanor). 

While Badpenny sits in jail awaiting the disposition of her case, many of the jailers treat her with indifference.  When Badpenny requests to use the phone, take a shower, or get commissary, the jailers disregard her request.  Badpenny noticed that the jailers did not treat the other prisoners as poorly; she has worked with most of these jailers and cannot believe the jailers would mistreat her.  

Badpenny has no one to talk to except her cellmate, Heather Scandell, whose current arrest was for possession of cocaine and methamphetamines with intent to sell. Badpenny and Scandell have been in jail for weeks together in the same cell sharing life experiences and hopes for their future.

Everyday Scandell tries to get Badpenny to discuss her case, but Badpenny remains guarded; she will not discuss her criminal case with anyone but her attorney, Liddy Gate.  One day, after getting back from the law library, Scandell’s cot was empty, Badpenny asked the jailer where she went, the jailer told Badpenny not to worry about the other inmates.  Later that day, a jailer came to inform Badpenny that her attorney wants to talk with her, the jailer escorted Badpenny to the visitation room.

Liddy Gate told Badpenny to sit down she had some troubling news about her case.  Once Badpenny sat down, Liddy informed Badpenny that Heather Scandell, her cellmate has become a jailhouse informant for the prosecution against her.  Liddy said, “Scandell is going to get a reduced sentence in exchange for testifying against you.  Scandell spoke with the prosecuting attorney and explained how you got the confidential information from county and city where you where worked, and then gave it to Dreadford in exchange for methamphetamines.”

Badpenny told Liddy that none of what Scandell says is true; she has not talked to anyone about her case.  Badpenny asked Liddy, “Why is this happening to me?” 

Liddy told Badpenny, “I will do what I can to minimize the damage Scandell may cause.  I will see if we can get a plea deal instead of going to trial.”

Assignment:

This week, use the information from chapters 10 and 11 to complete a 2 to 3 – page paper, discussing the implications of prosecutors using jailhouse informants and the problems with wrongful convictions. Also, explain how the occupational subculture in corrections plays a role in how correctional officers treat or mistreat prisoners.
