Actions) to create 2 distinct personas. When designing systems, it is important to give conscientious thought to who your users are and how they will use the system. In particular, you will want to pay attention to the new functionality / design that your users will interact with. Although the template provided is available, feel free to use your own creativity in this space and you are not required to go with this format.

Tips

Tip 1: Constructing personas is a requirements gathering exercise, similar to Blog 2. Meaning, you want to identify who your users are, what their needs are and how your design can satisfy those needs.

Tip 2: Identify 2 different types of users for your system. My goal is to redesign SocialXYZ, so I might create a persona to represent a Student and another to represent an Instructor. I do this because these two users will likely have different needs for my system.

Tip 3: Make your personas as close to your target demographic as possible. Remember, the persona is not a single user but an archetype, or representation of many users who share similar needs. In my case, I might want to target a traditional college junior, who I named Mack Stringer, who is 21, lives on campus and is a Computer Science major. 

Tip 3: Next, you will want to flesh out your persona so it appears as a generalized, but specific user of your system, with goals and background information that directly link to the goals and background of your system. For example, Mack, may use social networking technologies such as Facebook, Instagram and Snap, but has never used them for professional or academic reasons. However, Mack sees that it may be useful to find other users who are working part-time and are CS majors. Therefore, he might want to use SocialXYZ to initiate study groups or attend CS seminars. 

Tip 4: The final Box, labelled Project Goals, are specific design considerations you make that meet the needs of your persona. In my example, I want to redesign SocialXYZ to incorporate geolocation services to allow Mack to locate other CS majors on campus.

Job Attitudes: A Crisis in Nursing

 

 Los Rayos del Sol Medical Center is hospital and surgery center located in Florida.* A facility with 500 beds, it has recently partnered with the Mayo Clinic. Los Rayos’s is experiencing high turnover.  The nurse average turnover rate is 14% for hospitals[i], while Los Rayos has a turnover rate of 21%. New graduate nurses turnover at a rate of 27% within their first year, with an additional 37% of those new nurses wanting to leave.[ii] At Los Rayos, new nurse turnover is 40%. The hospital spends an average of 13 weeks[iii] to fill a vacant position and thousands of dollars per hire.[iv]Turnover often leaves units understaffed which creates poor patient experiences and lower quality of care.

 Why are the nurses leaving? Los Rayos strives to provide the highest quality in patient care, but it also has to manage costs and comply with government regulations. Thus over the last 10 years Los Rayos has made a number of changes. A summary of those changes are outlined below.

  

o  Ten years ago, Los Rayos changed the staffing model. All units had 2 licensed nurses and a housekeeper. Housekeepers were minimum wage staff that helped the nurses do things like wash linens and stock the nurses’ station with basics. These tasks can take a lot of time away from the normal nurses’ job duties of doing rounds, required charting, administering doctor’s orders, and helping patients. Los Rayos promoted the housekeepers to health techs which were supposed to do more patient care tasks, but most were not equipped with the skills. At the same time, Los Rayos reduced the number of nurses by one. This raised staffing ratios from 12 patients to one nurse to 24 patients to one nurse. 

o  Eight years ago, the Los Rayos cut the annual employee picnic and Christmas party in order to save costs. 

o  Five years ago, Los Rayos expanded nurses jobs to engage in activities like cost cutting and quality control.  It required nurses to provide 3-5 cost saving ideas per year or they would be negatively evaluated on their performance appraisals. 

o  Four years ago, the firm put a cap on each position’s wage brackets, which resulted in nurses with greater than 12 years of service to not receive raises. 

o  Three years ago, Los Rayos removed the intake coordinator position from all units except the ER and laboratory. This means that unless a patient is admitted to the hospital in the ER or immediately after laboratory testing, the nurse(s) on the unit have to complete the admission paperwork when the patient is brought to the unit. 

o  Two years ago, Los Rayos began using tablets to for patients charts and dispensing medicine. To prevent drug theft, medicine carts were equipped with a new security system that requires nurses to scan the patient’s hospital bracelet with the tablet, select the medication, and confirm the order before the tablet will send the information to the cart and unlock the needed medicine. The process often causes frustration to both nurse and patient as the tablet would not always stay connected to the med-cart or would have other errors. 

o  A year ago, Los Rayos began requiring all its nurses to take turns developing, planning, and presenting continuing education courses to reduce training costs. The nurses did not receive any additional compensation for the training they developed, nor were they given any recognition or non-monetary rewards.

o   Nurses are required to complete 60 hours of continuing education annually in order to for license renewal. Los Rayos used to pay for the nurses to attend these trainings and hourly wage, however that has been removed as well.

o   Six months ago, Los Rayos changed from 8 hour to 12 hour shifts to reduce costs and to allow patients to “be closer to their caretakers.” However, patients from the maternity and geriatrics wards have complained about only seeing nurses at the start and end of shift. Some employees like 12 hours shifts; however, most employees agree these shifts are exhausting and the nurses often state they don’t have the time and energy to “go the extra mile” for colleagues and patients. 

Discussion Questions

1.  How have the changes Los Rayos made affecting nurses attitudes? What problems to the business may poor nurse attitudes cause?

2.  Which of the job attitudes from the chapter do you feel is the biggest contributor to nurse turnover? The smallest contributor? Why do you think so?

3.  Los Rayos has tried to make the nurses’ jobs more rewarding by allowing them to be creative and have input into the organization (the CE trainings they have to develop and the cost saving ideas they have to put forth). However, the nurses do not perceive these additional duties to be a form of empowerment as intended. A) What can Los Rayos do to correct this misperception? B) What would have been a better way to engage nurses (and other staff) to help with the organizations issues (increasing costs, lower revenues, less than ideal patient satisfaction, and increasing government regulation)?

  1. How many sex offenders live in your zip code (If you don’t have any, which isn’t likely, pick a friend’s zip code)
  2. What are some of the crimes they were convicted of committing? What were some of the victims’ ages?
  3. How do you feel about them living in your area?
  4. How do you feel about everybody knowing their criminal past if they’ve reformed, and are just trying to live in peace?
  5. Should this type of information be more easily available (for other crimes, for example) to help us protect ourselves, or less easily available to help former offenders reenter society?

Among on the National Core Competencies for Nurse Practitioners I choose to elaborate on Leadership competency.  As highlighted in the Nurse Practitioner Core Competencies by (NONPF), the Nurse Practitioner Provides leadership to foster collaboration with multiple stakeholders (e.g. patients, community, integrated health care teams, and policy makers) to improve health care (NONPF, 2012) Leadership qualities was displayed in working through previous DNP course NU 740, by collaborating with various disciplines while gathering data to support my SPP. For example, if X = (1,1,0,0) and Y = (1,0,0,1) then their Hamming distance is 2 since X and Y differ in their second and fourth positions counting from left to right. The Hamming distance is a useful tool in various subfields of computer science including communication networks where it captures how much two binary vectors differ. In the above example, X = (1,1,0,0) may comprise four bits transmitted by a sender whereas Y = (1,0,0,1) are the actual bits received by a receiver. The Hamming distance, d(X,Y) = 2, indicates that two bits of X flipped — i.e., changed their value from 0 to 1, or 1 to 0 — while traveling from sender to receiver.   • From Hero to Humble (this is just one item that is selected from exhibit 1.2e for discussion….all the others are important to but this one along with the cowboy code are the things to focus on that you need to respond to in the form a discussion board for week 1)

 Another shift is the move from celebrating the “leader-as-hero” to recognizing the hard-working behind-the-scenes leader who quietly builds a strong enduring company by supporting and developing others rather than touting his or her own abilities and successes. One reason for the shift from hero to humble is that it is less and less realistic for an individual leader to meet all the challenges a team or organization faces in a complex and rapidly changing world. Another is that ambitious, highly self-confident, charismatic leaders have been at the forefront of some of the ethical scandals and business failures of recent years.

Consider This:

These leaders are characterized by an almost complete lack of ego, coupled with a fierce resolve to do what is best for the organization. They accept full responsibility for mistakes, poor results, or failures, but they typically give credit for successes to other people.
