76-year-old Iranian Male

BACKGROUND

Mr. Akkad is a 76 year old Iranian male who is brought to your office by his eldest son for “strange behavior.” Mr. Akkad was seen by his family physician who ruled out any organic basis for Mr. Akkad’s behavior. All laboratory and diagnostic imaging tests (including CT-scan of the head) were normal.

According to his son, he has been demonstrating some strange thoughts and behaviors for the past two years, but things seem to be getting worse. Per the client’s son, the family noticed that Mr. Akkad’s personality began to change a few years ago. He began to lose interest in religious activities with the family and became more “critical” of everyone. They also noticed that things he used to take seriously had become a source of “amusement” and “ridicule.”

Over the course of the past two years, the family has noticed that Mr. Akkad has been forgetting things. His son also reports that sometimes he has difficult “finding the right words” in a conversation and then will shift to an entirely different line of conversation.

SUBJECTIVE

During the clinical interview, Mr. Akkad is pleasant, cooperative and seems to enjoy speaking with you. You notice some confabulation during various aspects of memory testing, so you perform a Mini-Mental State Exam. Mr. Akkad scores 18 out of 30 with primary deficits in orientation, registration, attention & calculation, and recall. The score suggests moderate dementia.

MENTAL STATUS EXAM

Mr. Akkad is 76 year old Iranian male who is cooperative with today’s clinical interview. His eye contact is poor. Speech is clear, coherent, but tangential at times. He makes no unusual motor movements and demonstrates no tic. Self-reported mood is euthymic. Affect however is restricted. He denies visual or auditory hallucinations. No delusional or paranoid thought processes noted. He is alert and oriented to person, partially oriented to place, but is disoriented to time and event [he reports that he thought he was coming to lunch but “wound up here”- referring to your office, at which point he begins to laugh]. Insight and judgment are impaired. Impulse control is also impaired as evidenced by Mr. Akkad’s standing up during the clinical interview and walking towards the door. When you asked where he was going, he stated that he did not know. Mr. Akkad denies suicidal or homicidal ideation.

Increase Exelon to 3 mg orally BID

Guidance to Student
Cholinesterase inhibitors can take months to demonstrate any stabilization in the degenerative course of Alzheimer’s disease. Since the client has had no side effects, it would be prudent to consider increasing the Exelon dose to 3 mg orally BID. Recall that the target dose of Exelon is 12 mg orally daily in divided doses, or a transdermal patch delivering 9.5 mg daily could be used. Slow titration of the drug toward a therapeutic dose will decrease the risk of side effects. These should be teaching points directed toward the client and his son.
You could maintain the current dose of Exelon and reevaluate at the next office visit, but since the client is having no side effects and the client has been on the current dose for at least 4 weeks, it would probably be advisable to increase at this time to facilitate the goal of arriving at a therapeutic dose of the medication.
It may be early to augment with Namenda. Maximization of the Exelon dose should first occur, then augmentation with an NMDA receptor antagonist would be appropriate, but Namenda should be started at 5 mg orally daily, and then titrated up to a maximum dose of 10 mg orally BID. Doses over 5 mg orally daily should be divided into two doses, or the drug can be switch to an extended release preparation.
Finally, it is important to note that changes in the MMSE should be evaluated over the course of months, not weeks. The absence of change in the MMSE after 4 weeks of treatment should not be a source of concern.

Collaboration & communication action plan

As a special education teacher, you will be expected to provide support and guidance to administration, teachers, and staff regarding issues that may arise concerning how to meet the needs of individuals with disabilities outlined in IEPs. Serving as a collaborative resource to colleagues promotes the well-being of individuals with disabilities, the main goal of a special education teacher. Additionally, it is vital to provide support and guidance for students in the inclusion setting who will be working and collaborating with non-exceptional peers and general education teachers.

John is currently a ninth-grade student who has been identified as having an emotional/behavioral disorder, specifically in the area of obsessive-compulsive disorder (OCD). During his annual IEP meeting, John’s IEP team reviewed recent test scores and determined he qualified for the gifted program in the area of English language arts (ELA). John will continue to participate in the inclusion classroom for the remainder of his courses, and be placed in the gifted classroom for ELA in the second semester of the school year.

As John continues to be successful in all of his inclusion classes, he is having difficulty in his new ELA class. The gifted teacher, Ms. Griedl, was not able to be present during the IEP meeting, and John is the first student with an IEP she has had in her class. John has stated that being the only Hispanic student in Ms. Griedl’s gifted class has made him feel uncomfortable at times. He says Ms. Griedl does not include him in the classroom conversations and when she speaks to him, it is abruptly. John says she does not treat the other students in the same manner.

After one month of John being in the gifted class, Ms. Griedl has requested that a paraprofessional be present to support John. The IEP team determined that this was not necessary and that John should be able to be successful in the gifted classroom without a paraprofessional. The principal, Mr. Fleming, did a walk through observation of Ms. Griedl’s gifted classroom and he did notice that John was isolated in a corner, and the teacher and other students in the class did not interact with him. In addition, Mr. Fleming received a phone call from John’s parents who reported that Ms. Griedl had told John she felt he did not belong in the gifted class. John no longer wants to be in the class because he does not feel welcome.

 When someone loses someone they are going through so many emotions and making so many decisions. Everyone griefs differently and people will always have something to say about how they think a person should handle a situation. In the video the women talks about different situations where someone has lost a loved one and people on the outside believe that they should move on or that the way they are handling the situation is not healthy (Tedx, 2017). As a human service professional we should know that there are going to be people in a grieving person’s life who has a say and we need to be their advocate. Listening to their feelings and keeping them on a healthy course is the priority.

Great post this week! This writer would like to agree with your statement that every culture has a different reaction to death. This writer believes that every culture responses differently to death because of the many different traditions, beliefs, and religions that guide cultural followers to their ideology of death and the process of a deceased loved one. According to the Institute of Medicine (1984) every culture has rituals to mark the death of individuals and to assist surviving loved ones with their loss. With this being said the Institute of Medicine (1984) states that ethnic groups such as African burials differ greatly from Japenese burials in terms of grievance, funeral services, and traditional burial sites. With the many different cultures and the many different bereavement traditions, it may seem difficult for a professional to keep up with or even have the knowledge to recognize the differences. Although, the Institute of Medicine (1984) states that mental health professionals should use great caution in interperting the bereavement experiences in all clients. This caution should be taken because of the possibility that the norms for these groups may differ from one’s own or from those in a mainstream culture (Institute of Medicine, 1984). For example, this writer is used to quiet and sad funerals overall, whereas the African tradition is more of an upbeat process where it is encouraged to be loud and be heard as well as allowing loud music during the processes (Insititute of Medicine, 1984). Therefore, mental health professionals should make an effort to be aware of and accomodating  to alternative cultural practices for handling the process of death and to reduce the potential for cross-cultural miscommunication (Institute of Medicine, 1984). This writer believes that using caution and trying to be a more aware professional in the field will allow her to recognize when cultural differences will be presented during sessions.

Next explore South’s Online Library and conduct a search to locate a minimum of three nursing research articles that have addressed the problem you have identified. The literature may not be older than 5 years. After reading the literature respond to the following:

  1. Note your role specialization and briefly describe the problem you are interested in researching and why this is of interest to you.
  2. Discuss your library search strategies and briefly summarize the literature you reviewed. Focus your discussion on the “problem” and how the literature you identified provides evidence of the existence of the problem.
