In the media introduction to this module, it was suggested that you as a nurse have an important role in the Systems Development Life Cycle (SDLC). With a focus on patient care and outcomes, nurses may not always see themselves as contributors to the development of new systems. However, as you may have observed in your own experience, the exclusion of nurse contributions when implementing systems can have dire consequences.

In this Discussion, you will consider the role you might play in systems development and the ramifications of not being an active participant in systems development.

To Prepare:

• Review the steps of the Systems Development Life Cycle (SDLC) as presented in the Resources.

• Reflect on your own healthcare organization and consider any steps your healthcare organization goes through when purchasing and implementing a new health information technology system.

• Consider what a nurse might contribute to decisions made at each stage of the SDLC when planning for new health information technology.

Describe what you believe to be the consequences of a healthcare organization not involving nurses in each stage of the SDLC when purchasing and implementing a new health information technology system. Provide specific examples of potential issues at each stage of the SDLC and explain how the inclusion of nurses may help address these issues. Then, explain whether you had any input in the selection and planning of new health information technology systems in your nursing practice or healthcare organization and explain the potential impacts of being included or not in the decision-making process. Background Information

Many of the staples in Jesus’ time (grains, flour, fruits, fish, meat) were sold by the pound, and the merchants owned their own scales. These scales were not accurate so the buyer paying for a pound of fish might get only 14 or 15 ounces. Jesus was incensed by what he saw in the temple which was supposed to be a place of worship but had become a marketplace. Seeing the deceptions, He called out merchants who were for all practical purposes lying to and stealing from their customers.

“Name two different methods for evaluating evidence. Compare and contrast these two methods”

When researching there is a common course from the time of research to evaluating the results of the research. Two different types of data analysis within the statistical realm are Inferential and descriptive statistics. Both are a way to interpret scientific evidence, but have differing methods of reporting the evidence.

Descriptive Statistics

Descriptive statistics allows the researcher to take a sample group and graph the data to understand that specific set of observations from only the information they have witnessed to describe the results (Frost, 2021). This type of research analysis allows one to achieve more insight and visualization of the results of only what has been studied or seen, instead of assuming information about a larger population (Frost, 2021).

Inferential Statistics

Inferential statistics uses data from a small sample and makes inferences from them, applying the results to a larger population; due to this results must be extremely accurate (Frost, 2021). With this type of sampling there is a margin for error because if the sample does not accurately reflect the population being researched it will skew the results (Frost, 2021).

While both types of research data analyze research results, descriptive statistics will be more accurate since it is only reporting data true to its research and sample size, and nothing outside of that. On the other hand, inferential statistics applies results from a smaller sample size to a bigger population, which may produce variations in results.

There are several methods of evaluating evidence methods, but the two most common methods of evaluating evidence are Meta-Analyses and Systematic Review. A systematic review are literature reviews that summarize evidence by identifying, selecting, assessing, and synthesizing the findings of similar but separate studies (Agency for Healthcare Research and Quality [AHRQ], 2018). Meta-analysis is the combined form of many data from different studies. The systemic review is a meticulous type of research whereas the meta-analysis is a statistical process which combines studies results and test the hypotheses. The systemic review is also known as secondary research and meta-analyses is the process of systemic research. A systematic review summarizes the results of available carefully designed healthcare studies (controlled trials) and provides a high level of evidence on the effectiveness of healthcare interventions” (Cochrane Consumer Network, n.d.,). This type of review is regarded as the best research evidence (Helbig, J. 2018).

The critical appraisal is used to identify the strengths and weaknesses of the article selected for the literature review. When evidence is gathered, the researcher performs a critical appraisal of the “study to ensure its credibility and clinical significance. It is important to determine not only what was done, but how well it was done” (Connor, 2014).

Both the Systematic Reviews and Meta-Analyses are considered the highest quality of evidence for clinical decision making and can be used above all the other methods of evaluating evidence. Both methods for evaluating evidence are similar because they involve the collection of data from different sources and summarizing all the evidence and results of the studies.

While systematic review collects and summarizes all the empirical evidence, the meta-analysis uses statistical methods to summarize the results of the studies. Meta-analysis is a statistical method used to combine the numerical results from such studies if it is possible to do. On the other hand, systematic review is a formal, systematic, and structured approach to review all the relevant literature on a topic. The other difference is, the rationale for Meta-analysis is that through the combination of samples from different studies the overall sample size is increased, while the rationale for systematic reviews is that when data is combined from different sources a greater reliability would be gained.

When performing a systematic literature review or meta-analysis, and the quality of studies is not rigorously evaluated or if proper methodology is not strictly applied, the results can be biased, and the outcomes can be incorrect. However, when systematic reviews and meta-analyses are properly implemented, they can yield powerful results.

Jean Watson Theory

Jean Watson’s Theory of  Human Caring/Caring Science is one theoretical framework used throughout the USU College of Nursing courses. The practice implication of Watson’s Human Caring Theory evolves our thinking and approaches to patient care from a mindset of carative (cure) to one of caritas (care). The core principles/practice are founded on a:

  • Practice of loving-kindness and equanimity
  • Authentic presence: enabling deep belief of other (patient, colleague, family, etc.)
  • Cultivation of one’s own spiritual practice toward wholeness of mind/body/spirit—beyond ego
  • “Being” the caring-healing environment
  • Allowing miracles (openness to the unexpected and inexplicable life events)

Some individuals are comfortable framing their practice with Watson while others prefer different theories or a collection of theories. TESLA – Strategy Formulation and Implementation

Tata Motors and Tesla are two of a large number of firms that have taken steps to flatten their hierarchical structures. Tata, an India-based automaker, cut half of the levels in the organization, going from 12 to 6 levels of management, after the firm hired a consultant to help improve its structure. Similarly, Tesla announced in 2018 that it would flatten its structure “to improve communication.” These firms are not alone in their drive to cut out layers of management. One survey concluded that 93 percent of polled firms indicated they intended to flatten their organization in the near future.

Proponents of flat leadership structures argue they offer cost, flexibility, and creativity benefits. First, flatter organizational structures are designed to be more responsive and faster acting. By cutting out layers of management that analyze and approve initiatives developed by lower level employees and teams, the firm empowers these teams to act quickly. Second, flatter organizations allow workers and teams to have autonomy, a desirable attribute for younger employees. Third, by cutting out layers of management, the firm reduces the cost of a large administrative task. Fourth, feedback loops through the organizational structure are quicker and shorter. Top level managers are closer to line employees and the market, allowing them to have better insight into the dynamics within the firm and the market.

However, critics have argued that flattening organizations face unintended negative consequences. First, management attention becomes stretched. With the expanding scope of their responsibilities, the remaining managers find that the bulk of their time is spent on communication. A number of studies have found that managers in flat organizations spent 80 to 90 percent of their day communicating, leaving little time for other tasks. This can lead to “interaction fatigue” on the part of managers. Second, middle-level managers often serve as critical negotiators between different units in the firm and between front-line employees and top managers. In cutting these managers, some firms have found that frictions between units have increased. Third, the removal of middle manager positions can be demotivating for some employees since there are reduced opportunities for advancement into the ranks of management. Echoing these concerns, Harvard researchers Thomas DeLong and Vineeta Vijayaraghavan argue that long serving middle managers are a competent and steadying influence in the firm. They are experts at how the firm operates and can insulate the firm from impulsive decisions that come from either the top or the bottom.

These conflicting perspectives suggest that organizations seeking to flatten their structures need to be aware of both the potential costs and benefits of the action. But the ultimate value of the action appears uncertain.

CASE STUDY ONE: COLLEEN M.

Colleen M. is a 15-year-old Caucasian girl. She has lived in the state for her entire life. Her father abandoned the family when she was an infant and her mother is currently in a drug rehabilitation facility. Colleen has been living on a farm with her aunt. Although she has no prior contact with the law, she admits to having run away from her aunt in the past, though she always returned on her own.

Colleen’s current trouble is centered on a neighbor’s dispute about her dog. After the dog bit the neighbor, the neighbor threatened and attempted to take the dog to animal control. When the neighbor later found Colleen and the dog trespassing on her property, the neighbor verbally threatened to “get” them both. Apparently, the neighbor set fire to a scarecrow on her property to emphasize her point. 

At this point, Colleen threw a nearby bucket of liquid on the neighbor. The liquid was later determined to be a pesticide. The pesticide burned the skin on contact and triggered an extreme allergic reaction in the neighbor, resulting in her death. Colleen was charged with involuntary manslaughter and was determined delinquent by the court. Colleen claims that she was simply attempting to douse the fire.

Coincidentally, a few weeks prior, Colleen had been a passenger of a motor home that had collided with a vehicle driven by the neighbor’s sister. This accident resulted in the neighbor’s sister’s death, though it was ruled an accidental homicide. Witnesses report that Colleen took a pair of fancy shoes off the feet of the corpse and immediately put them on her own feet. Colleen defends her actions by claiming that an observer told her to do this. 

At the time of her arrest, Colleen’s blood tested positive for opiates. Colleen claims that while traveling with three homeless men, they unknowingly stumbled upon a field of poppies and that before they knew it; the intoxicating aroma caused her to pass out. Colleen claims not to have a drug problem. Colleen states that all she wants to do now is go home and that she misses her Aunt Emily.

CASE STUDY TWO: XANDER L.

Xander L. is a 17-year-old African American male and documented gang member. His prior juvenile adjudications include purse snatching, breaking and entering, and drug possession. His first juvenile adjudication occurred when he was 13 years old. He has served a year of custody in the juvenile correctional facility and has been placed on probation twice previously. His latest arrest is for possession of a concealed weapon, to which he has pled guilty. 

Xander lives with his mother in the housing projects. He reports no contact with his father. His mother presently works two jobs. Although she is supportive of him, and Xander reports a close relationship with his mother, she does not approve of his conduct. In the past, she has reported him to the police. Xander indicates that he has dropped out of high school, but would like to get his general equivalency diploma to make his mother proud.
