In this assignment, you will calculate confidence intervals for the quantitative variables in the Heart Rate Dataset.

Steps

  1. Open the Heart Rate Dataset in Excel and identify the quantitative variables
  2. Make sure the data is sorted by category (e.g.male-at-rest, female at-rest, etc.)
  3. Use the Data Analysis tools of Excel to construct 95% and 99% confidence intervals for all 8 categories of the sorted quantitative variables.  Excel will calculate the margin of error given as the “confidence interval” in the output. Use this margin of error to create your 8 confidence intervals by both adding and subtracting it from the sample mean (calculated in unit 3).  This creates a range of values that is the confidence interval.
  4. Create a Word document, and use your calculated results to explain what the confidence intervals tell us. What do these confidence intervals tell us about our heart rate data?  How would you interpret the 95% and 99% intervals for resting heart rate?
  5. Compare the 99% and 95% intervals and explain why one is larger than the other even though we use the same sample mean value.

  Insomnia is one of the most common medical conditions you will encounter as a PNP. Insomnia is a common symptom of many mental illnesses, including anxiety, depression, schizophrenia, and ADHD (Abbott, 2016). Various studies have demonstrated the bidirectional relationship between insomnia and mental illness. In fact, about 50% of adults with insomnia have a mental health problem, while up to 90% of adults with depression experience sleep problems (Abbott, 2016). Due to the interconnected psychopathology, it is important that you, as the PNP, understand the importance of the effects some psychopharmacologic treatments may have on a patient’s mental health illness and their sleep patterns. Therefore, it is important that you understand and reflect on the evidence-based research in developing treatment plans to recommend proper sleep practices to your patients as well as recommend appropriate psychopharmacologic treatments for optimal health and well-being. 

Case: An elderly widow who just lost her spouse. 

Subjective: A patient presents to your primary care office today with chief complaint of insomnia. Patient is 75 YO with PMH of DM, HTN, and MDD. Her husband of 41 years passed away 10 months ago. Since then, she states her depression has gotten worse as well as her sleep habits. The patient has no previous history of depression prior to her husband’s death. She is awake, alert, and oriented x3. Patient normally sees PCP once or twice a year. Patient denies any suicidal ideations. Patient arrived at the office today by private vehicle. Patient currently takes the following medications: 

  • Metformin 500mg BID 
  • Januvia 100mg daily 
  • Losartan 100mg daily 
  • HCTZ 25mg daily 
  • Sertraline 100mg daily 

Current weight: 88 kg

Current height: 64 inches

Temp: 98.6 degrees F

BP: 132/86 

 

  • List three questions you might ask the patient if she were in your office. Provide a rationale for why you might ask these questions. 
  • Identify people in the patient’s life you would need to speak to or get feedback from to further assess the patient’s situation. Include specific questions you might ask these people and why. 
  • Explain what, if any, physical exams, and diagnostic tests would be appropriate for the patient and how the results would be used. 
  • List a differential diagnosis for the patient. Identify the one that you think is most likely and explain why. 
  • List two pharmacologic agents and their dosing that would be appropriate for the patient’s antidepressant therapy based on pharmacokinetics and pharmacodynamics. From a mechanism of action perspective, provide a rationale for why you might choose one agent over the other.

That is, what would be problematic with the use of this drug in individuals based on ethical prescribing guidelines or decision-making?
  • Include any “check points” (i.e., follow-up data at Week 4, 8, 12, etc.), and indicate any therapeutic changes that you might make based on possible outcomes that may happen given your treatment options chosen.

The babysitter said she had been playing with the children in the front yard when the patient ran after a ball that was rolling toward the street. She grabbed the patient’s left forearm and pulled him away from the street. Thereafter, the patient was irritable and holding his left arm close to his body with the elbow in a flexed position. On physical examination, the child has no bony tenderness, erythema, or swelling of the joints, but on passive motion, the child resists and cries in pain. Today, his vitals are as follows: weight 40.5 lbs, height 43.0 inches, BP 110/76, HR 100, RR 26, and Temperature is 98.6 F.

Step 1 – Read the assigned clinical scenario and using your clinical reasoning skills, decide on the diagnoses. This step informs your next steps.
Step 2 – Document the given information in the case scenario under the appropriate sections, headings, and subheadings of the SOAP note.
Step 3 – Document all the classic symptoms typically associated with the diagnoses in Step 1. This information may NOT be given in the scenario; you are to obtain this information from your textbooks. Include APA citations.

An Advanced Directive is a document in which a client who is competent is able to express wishes regarding future acceptable health care. The document may include decisions including the desire for extraordinary lifesaving measures, such as resuscitation, intubation, and artificial hydration, and nutrition. The advanced directive may also be used to designate another person to make decisions when the client becomes physically or mentally unable to do so. The Patient-Determination Act requires all clients admitted to a health care facility be asked if they have advanced directives.

Planning guides for seriously ill clients includes:

  1. Living will: Legal document that instructs health care providers and family members about what, if any, life-sustaining treatment an individual wants if at some time the individual is unable to make decisions.
  2. Durable power of attorney for health care: Legal document that designates another person to make health care decisions for the client when the client becomes unable to make decisions independently.

Nursing intervention in these types of situations is to ensure to communicate to the interdisciplinary health care team that the client has provided and advanced directive and that it has been placed in the client’s medical record. Clients who do not have advanced directives must be given written information outlining rights related to health care decisions.

Initial Discussion scenario:

Mr. Gomez is a 73-year-old man who is estranged from his wife. Since he prefers that she doesn’t make any medical decisions for him should the need arise, he has filled out an advance directive naming his daughter Elena as his health care proxy. He has suffered from diabetes for many years and is adamant that he does not want dialysis, even if it would sustain his life. He does, however, wish to have tube feeding for however long it will sustain him.

One evening while at home with his daughter, Mr. Suarez has a seizure. He is taken to the hospital immediately, but never regains consciousness and doctors do not expect him to do so. Elena contacts her mother Mr. Gomez’s estranged wife and she quickly joins her daughter at the hospital. They want to put him on dialysis (as his diabetes is worsening) and a feeding tube (as there is no other way for him to receive nutrients). His daughter informs the doctors of her father’s end-of-life wishes. The doctors warn her that without dialysis her father will pass away more quickly, even with the feeding tube. Mr. Gomez’s estranged wife informs Elena that she needs to follow the advice received from the doctors in order to save her father’s life. Elena says that although she is aware of the consequences, she wants them to carry out her father’s wishes.

The clients estranged wife immediately has her attorney file an order to stop Elena from following her father’s wishes that are in his advanced directive. She feels that even though they are estranged, she still should have the right to make decisions regarding Mr. Gomez’s care. This action delays Elena’s ability to honor her father’s wishes and the doctors feel that it is necessary to inform the hospital’s ethics committee about the ongoing issue.

Topic and Structure:

Multimodal Personal Narrative – Develop a multimodal document to both visually illustrate and verbally express a personal transformation.

Use a one-page newspaper or single-panel brochure format to present a personal narration of a transformation of your choice from a point in your childhood to today. You might discuss a career aspiration you had as a child, transition to the job you held as a teenager, and lastly, explain the profession you maintain today. Use connecting ideas to ensure you have a cohesive essay, which will ultimately (in the conclusion) explain what you learned from this transformation. For example, how and why did you go from point A (job 1) to point B (job 2) and then on to point C (job 3)? Or, you might consider illustrating a transformation based on a way of life or philosophy.

Implement three photos to represent your ideas, feelings, etc., at the three focal points in your life. “Strategic Management and Strategic Competitiveness Overview”

 

Choose one public corporation in an industry with which you are familiar. Research the company on its own website, the public filings on the Securities and Exchange Commission’s Filings & Forms page, the Lexis Advance database, and any other sources you can find. The annual report will often provide insights that can help address some of these questions.

Requirements

Write a 4–6 page paper in which you address the following:

  • Assess how globalization and technology changes have impacted the corporation you researched.
  • Apply the industrial organization model and the resource-based model to determine how your corporation could earn above-average returns.
  • Assess how the vision statement and mission statement of the corporation influence its overall success.
  • Evaluate how each category of stakeholder impacts the overall success of this corporation.

The specific course learning outcome associated with this assignment is the following:

Week 3 Strategic Management and  Strategic Competitiveness Assignment

Globalization

Thoroughly assess how globalization has impacted the public corporation you researched. Provide a thoughtful and well researched response. Consider for example, how your chosen company has been involved in expanding globally, how it has been impacted by global competition, and the global economy. Do not simply define the term globalization. If your company has locations in other countries, do not just state that your corporation has locations in other countries or simply list the various countries in which the company does business. You need to assess the impact globalization has had on your selected corporation. 

  

Technology

Thoroughly assess how technological changes have impacted the public corporation you researched. Provide a thoughtful and well researched response. Consider for example, how the company has been impacted by cloud computing, social media, crowdfunding, program apps, email, texting, websites, mobile, automation, robotics, IOT (Internet of Things), AI (Artificial Intelligence), e-commerce, data and analytics, etc. Research how the company may have been impacted by diffusion and disruptive technologies as explained in the textbook. Do not simply define the term technology. You will need to assess the impact changes in technology have had on your selected corporation. 

  

Industrial Organization Model

Thoroughly apply the industrial organization model to determine how your corporation could earn above-average returns (i.e. revenue). This model is based upon the corporation’s external environment which is anything outside of the corporation that can influence or impact its business (macro)/operations (micro). Your application should walk through the components the model suggests are needed to earn above average returns.  

  

Resource-Based Model

Thoroughly apply the resource-based model to determine how your corporation could earn above-average returns (i.e. revenue). Consider the corporation’s unique resources (corporate culture, land, location, equipment, brand, reputation, trademarks, patents, etc.) and capabilities (skills, experience, etc.) that set it apart from its competition. Your application should walk through the components the model suggests are needed to earn superior returns 

  

Vision

Thoroughly assess how the vision statement of the corporation influences its overall success. Include the actual vision statement for your chosen company (be sure to quote and cite your source).  

  

Mission

Thoroughly assess how the mission statement of the corporation influences its overall success. Include the actual mission statement for your chosen company (be sure to quote and cite your source). Regarding mission in your assessment of your selected corporation’s mission statement. Do not simply provide a definition of mission or make general statements.  

  

Stakeholders

Thoroughly evaluate how each category of stakeholder, described in the textbook, impacts the overall success of your selected corporation. Do not Google “stakeholders” or simply provide a definition or list of stakeholders. You must provide an evaluation that demonstrates your understanding of each classification of stakeholders.
