Question: What do we learn from 2016? What did the presidential election of 2016, Donald Trump’s victory, and the rise of white nationalism in popular culture teach us about racial attitudes in the US that we didn’t already know (if anything)? Are there existing theories of racial attitudes in politics that are due for a re-assessment, or that should be discarded, in the wake of this election? Or was the role of racial attitudes in 2016 similar to other past elections? Either way, describe the evidence you’re relying on to make your argument, and be sure to cite sources you have read in this module (or others) and briefly describe their theories as you use them. 

       Answer: The election of Donald Trump revealed many low key racists in America. It also gave many white supremacy room to be openly racist because of how comfortable they felt with Donald Trump becoming president. We all know racism existed in America for centuries. It just became so blatant and normalized when Donald Trump became president. White nationalist weren’t scared to hide it anymore because the most powerful person in the country was just like them. The Red MAGA hat became like a symbol for racism and white supremacy. It gave Trump supporters the power to be openly racist and prejudiced. It just goes to show how America hasn’t changed as much as people hoped. Racism is still very present in society and doesn’t seem to be going away. I think Race was a big topic of the elections. Everyone is wanted to see if Donald Trump really supported white supremacy or if he would speak against it. In the I Am Not Your Negro film it showed how the decision to integrate came with racists gathering together to jeer black school children. The people were comfortable enough to do this because the governor of the integrated state allowed it and even told the people to remove their children from the school. It reminded me of how Donald Trump also used his political platform to tell his supporters to storm the Capitol.

Read the following case study.

Ben Davis had just completed an intensive course in Statistical Thinking for Business Improvement, which was offered to all employees of a large health maintenance organization. There was no time to celebrate, however, because he was already under a lot of pressure. Ben works as a pharmacist’s assistant in the HMO’s pharmacy, and his manager, Juan de Pacotilla, was about to be fired. Juan’s dismissal appeared to be imminent due to numerous complaints, and even a few lawsuits over inaccurate prescriptions. Juan now was asking Ben for his assistance in trying to resolve the problem, preferably yesterday!

“Ben, I really need your help! If I can’t show some major improvement or at least a solid plan by next month, I’m history.”
“I’ll be glad to help, Juan, but what can I do? I’m just a pharmacist’s assistant.”
“I don’t care what your job title is; I think you’re just the person who can get this done. I realize I’ve been too far removed from day-to-day operations in the pharmacy, but you work there every day. You’re in a much better position to find out how to fix the problem. Just tell me what to do, and I’ll do it.”
“But what about the statistical consultant you hired to analyze the data on inaccurate prescriptions?”
“Ben, to be honest, I’m really disappointed with that guy. He has spent two weeks trying to come up with a new modeling approach to predict weekly inaccurate prescriptions. I tried to explain to him that I don’t want to predict the mistakes, I want to eliminate them! I don’t think I got through, however, because he said we need a month of additional data to verify the model, and then he can apply a new method he just read about in a journal to identify ‘change points in the time series,’ whatever that means. But get this, he will only identify the change points and send me a list; he says it’s my job to figure out what they mean and how to respond. I don’t know much about statistics — the only thing I remember from my course in college is that it was the worst course I ever took– but I’m becoming convinced that it actually doesn’t have much to offer in solving real problems. You’ve just gone through this statistical thinking course, though, so maybe you can see something I can’t. To me, statistical thinking sounds like an oxymoron. I realize it’s a long shot, but I was hoping you could use this as the project you need to officially complete the course.”

“I see your point, Juan. I felt the same way, too. This course was interesting, though, because it didn’t focus on crunching numbers. I have some ideas about how we can approach making improvements in prescription accuracy, and I think this would be a great project. We may not be able to solve it ourselves, however. As you know, there is a lot of finger-pointing going on; the pharmacists blame sloppy handwriting and incomplete instructions from doctors for the problem; doctors blame pharmacy assistants like me who actually do most of the computer entry of the prescriptions, claiming that we are incompetent; and the assistants tend to blame the pharmacists for assuming too much about our knowledge of medical terminology, brand names, known drug interactions, and so on.”
“It sounds like there’s no hope, Ben!”

Assuming the role of Ben Davis, write a three to four (3-4) page paper in which you apply the approach discussed in the textbook to this problem. You’ll have to make some assumptions about the processes used by the HMO pharmacy. Also, please use the Internet and / or Strayer LRC to research articles on common problems or errors that pharmacies face. Your paper should address the following points:

  1. Develop a process map about the prescription filling process for HMO’s pharmacy, in which you specify the key problems that the HMO’s pharmacy might be experiencing. Next, use the supplier, input, process steps, output, and customer (SIPOC) model to analyze the HMO pharmacy’s business process.
  2. Analyze the process map and SIPOC model to identify possible main root causes of the problems. Next, categorize whether the main root causes of the problem are special causes or common causes. Provide a rationale for your response.

Getting Started

There is an order to our world. Without order, the universe would be nonexistent, as many scientists are now understanding. For those that work in the world of computing, we enjoy the ability to use technology because there is order at the smallest levels of matter, the atomic level. The order is a very strict scientific principle that we are able to harness matter to represent data. In Christianity, the Bible mentions order in the creation of the universe: “Then God said, ‘Let there be lights in the expanse of the heavens to separate the day from the night, and let them be for signs and for seasons and for days and years’” (Genesis 1:14, New American Standard Bible).

Gender Dysphoria

Transgender is the term used to mean that individual sex assigned during birth based on the external genital does not fit their gender identity. These kinds of people usually experience gender dysphoria, which is one of the psychological distresses associated with the incongruence between one’s gender identity and the sex they were assigned during birth. Gender dysphoria usually starts at the beginning of childhood, but in some cases, individuals may not experience it until after puberty and even much later in their stages of life. People with transgender may pursue different domains of gender affirmation, which includes: surgical affirmation, medical affirmation, legal affirmation, and social affirmation (Lindley, 2020). Not all people with transgender issues will desire to have all these domains of gender affirmation as these are highly individual and personal decisions.

Medical treatment for gender dysphoria

Medical treatments for this condition may include hormone therapy, such as masculinizing hormone therapy or feminizing hormone therapy. Surgery includes masculinizing surgery or feminizing surgery, which intends to change chest or breasts, internal genitalia, external genital, body contouring, and facial features.

Some people used hormone therapy to seek maximum masculinization or feminization. Other people find relief from gender dysphoria by using hormones to maximize secondary sex characteristics such as facial hair and breasts. These kinds of treatments are based on individuals’ goals and the evaluation of the benefits and risks of the medication use, presence of any other conditions, and consideration of individual’s economic and social issues (Zucker, 2018). Most of these gender dysphoria individuals find surgery the most effective and necessary procedure that relieves their condition.

The World Professional Association for Transgender Health gives the following procedure for the surgical or hormonal treatment for people with gender dysphoria.

1. Persistent and a well-documented gender dysphoria

2. The capacity to make consent and an informed decision for the treatment

3. The aged majority of a given country and when the patient is young need to follow the standard of care for the adolescents and children.

Behavioral health treatment

This treatment aims to improve the individual’s psychological well-being, self-fulfillment, and quality of life. This kind of treatment does not aim to alter an individual’s gender identity, but it aims to explore gender concerns and find ways to reduce gender dysphoria. The main goal here is to help the gender-nonconforming and transgender individuals become more comfortable with the situation as gender identity expressions enable success in their education, work, and relationships. This therapy can also address any other mental health concerns (Kaltiala-Heino, 2018).

The therapy can be individual, family, group, or couple, which will help the individual explore and integrate on their gender identity and accept themselves. It will address the emotional and mental impact of the minority stress and build on a support network. To achieve these entire, one needs to develop a plan to address legal and social concerns related to the transition and coming out to the loved ones, close contacts, colleagues, and friends. This will make one more comfortable by expressing their gender identity. It will also explore healthy sexuality and make the right decision on medical treatment options and ensure an increased quality of life and well-being.

Therapy might be needful and helpful during many stages in everyone’s life. The pre-treatment of behavioral health evaluation is done by a doctor who is an expert and has experience in intersex and transgender health when needed for surgical and hormonal treatment of gender dysphoria.

Diagnostic Criteria for Gender Dysphoria

           According to the DSM V, Gender Dysphoria occurs when an individual believes that their gender identity does not correspond to the biological sex (APA, 2013).  These individuals prefer to be identified as the gender they personally identify with, not with the gender assigned at birth. Within their family and socially, they wish to be identified as either male or female depending upon their self-identification.  They experience personal distressed when they are viewed as the gender they were born with and they have a desire to have the body of the opposite sex (Sadock, Sadock & Ruiz, 2014).   Those with gender dysphoria often opt to have gender reassignment surgery and receive hormonal treatments (Kaltiala-Heino, 2018). At around age 2 or 3 gender identity is established (Sadock et. al., 2014). The criterion for Gender Dysphoria includes  two of the  following for at least 6 months: an incongruence between experienced gender and assigned gender, a desire to get rid of or prevent the development of primary and/or sex characteristics, a strong desire to have the sex characteristics of the other gender, a strong desire to be the other gender, a strong desire to be treated like the other gender, and a belief that one has the feelings and beliefs of the other gender and secondly,  distress or impairment in school, social settings and daily functioning (APA, 2013). Gender dysphoria may occur in children, adolescents and adults.

         Psychotherapy and Psychopharmacologic Treatment for Gender Dysphoria

For this disorder treatment options include psychotherapy, hormone therapy and gender reassignment surgery.  For children and adults, individual, group and family therapy is beneficial. With adults, hormonal and surgical options can be explored.  Since physicians who perform gender reassignment surgery require a letter from a mental health provider, individuals would participate in treatment so that they could get a psychiatric clearance for surgery. Nguyen et. al., (2018) suggest that prescribing gender affirming hormone therapy helps alleviate the mental distress related to gender dysphoria by reducing the symptoms of anxiety and depression Otherwise, these individuals would avoid mental health treatment. There is now an informed consent model in some clinics so that they could receive hormone therapy without a mental health professional’s involvement.  This is unfortunate since many adults and children with gender dysphoria have underlying psychiatric diagnoses such as anxiety , social anxiety , depression, risk of suicide, substance abuse and other self-harm behaviors (Sadock et.al., 2014). Mental health professionals do have an essential role in promoting the wellness of patients with gender dysphoria. Johnson, Shipherd and Walton (2020) emphasize the many services such as advocacy, coordination of care, and referrals for medical care and surgery.  Because of stigma and social anxiety, patients with this diagnosis may not seek help. Gabbard (2014) emphasized the importance of psychoeducation for spouses, parents and family members. A psychotherapist knowledgeable about gender dysphoria can be an invaluable resource.

Those who offer conversion therapy are not abiding by the position statements by the American Psychiatric Association or the guidelines established by the American Academy of Child and Adolescent Psychiatry (Sadock et. al., 2014).  Additionally, cognitive behavioral therapy (CBT) can be helpful for transgender or gender non-conforming (TGNC) patients by targeting social anxiety.

      Differential Diagnosis for Gender Dysphoria

 The patient must experience distress with their gender identity.  Those with psychosis who have delusional thoughts that they are the opposite gender may experience distress, however, once the psychosis is treated, the distress regarding gender diminishes.  Body dysmorphic disorder is a differential diagnosis in that some people may want to change gender body parts but they believe it is abnormal but they do not question their assigned birth gender.. Another differential diagnosis is transvestic disorder in which the individual identifies with their birth gender but is aroused by wearing the opposite gender’s clothing.

Case Study – Officer Tim Bronson

Officer Tim Bronson, badge #3293 was dispatched at 21:00 to 4321 West Jefferson on October 3rd. The purpose of the call was to respond to a reported burglary in the vicinity. Dispatch provided that alleged perpetrator had been described as a Caucasian male, approximately twenty to twenty-five years of age with a light build, wearing a blue jeans and a white zipper up hoody jacket. 

Officer Bronson observed a young man on a nearby street that seemed to match the description provided by dispatch. When Officer Bronson attempted to make contact with the young man, the young man fled down the street. Officer Bronson gave chase, and was able to capture and subdue the suspected burglar. The suspected burglar was identified as Carl Davidson. 

Officer Bronson placed Mr. Davidson under arrest for a violation of burglary statutes.

Post an interpretation of the case study’s conclusion that “the vocational rehabilitation intervention program may be effective at promoting full-time employment.”

Vocational training are programs that provides participants with required skills to complete specific jobs or tasks. The study that is trying to be determined amongst the case study is how the programs influence the participants and employment. While reviewing the Chi-Square case study, the results stated that the Vocational Rehabilitation Intervention Program might be effective at promoting full-time employment among recently paroled inmates. The charts indicate that out of 30 participants in the intervention group, 18 were full-time employees. In contrast, participants on the waitlist, only 6 were full-time employees. The study also indicates that usually, researchers use the alpha level of .05 to determine the significance of their study, however y using the Pearson Chi-Square test, the results were that researchers found the difference between the intervention and comparison groups where highly significant because the p value was .003 significant beyond the usual level. 

Describe the factors limiting the internal validity of this study and explain why those factors limit the ability to draw conclusions regarding cause-and-effect relationships.

Trochim explained that internal validity is the approximate truth about interferences regarding cause and effect or casual relationships. In the case study, it identified that the study was bias based on the preexisting differences amongst the participants and nothing else was used to help determine the validity. The study identified that more options should have been assessed. It should have looked at things such as if the paroled individual were able to maintain employment, earn living wages and satisfy other conditions of probation. If these factors was taken into consideration it should have been identified whether the participants have the skills or not to fulfill the duties. If the participants are skilled to maintain employment, earn living wages and other conditions of probation then it will determine that the works are trained to fulfill duties and if the participants fail they are not trained properly.

Huddles are such an important part of nursing – they relay pertinent information, increase safety, offer team building opportunities, promote better outcomes for the patient, and ultimately can reduce costs.

Here is your scenario:

Your nurse manager identifies that the staff on your rehabilitation unit, and particularly that you as the evening charge nurse, are resistant and oppositional to an important new patient safety strategy of implementing huddles to improve staff communication.

BRIEF SUMMARY OF CASE 

Paragraph 1  Your nurse manager meets with you, the evening charge nurse, (and with you he had previously begun a disciplinary action process because you refused to implement huddles on the shifts when you was in charge).   You now understands that you (and the staff) were wrong, and you begin to explore with the manager how better to implement huddles on your shift.
