The purpose of this assignment is to demonstrate your ability to evaluate one quantitative or qualitative nursing research article of interest from any refereed nursing journal. Writing a critical review of a journal article can help to improve your writing and analytical skills. By assessing the work of others, you develop skills as a critical reader and become familiar with the types of evaluation criteria necessary to translating the evidence to practice.Instructions:

  1. Write a 1-2 pages assignment.
  2. Choose a primary nursing research article of interest from a refereed nursing journal. Attach a copy of the article.
  3. Begin your critique by identifying the:
    • article’s title
    • author(s)
    • date of publication
    • name of the journal or other publication in which it appeared
    • Where and when was the article published?
    • In your introduction, you should also briefly describe the purpose and nature of the study and, if applicable, its theoretical framework.
    • If the paper was not published in a peer-reviewed journal, consider the credibility of the publication in which it appeared.
  4. If you are reviewing a research study, organize the body of your critique according to the paper’s structure. Start with a brief description and analysis of the strengths and weaknesses of the research design and methodology. For example:
    1. How does the method reflect other studies of the same topic? (sample, data collection, setting).
    2. What makes this method feasible?
    3. How realistic is it? How does the method address questions of validity?
    4. How does the researcher overcome the limitations of the method?
    5. Are there large limitations or minor ones?
    6. How will these limitations affect your ability to use this data to answer your research question?
  5. Describe the results by answering:
    1. How were the results interpreted?
    2. How were they related to the original problem (author’s view of evidence rather than objective findings)?
    3. Look at DISCUSSION. Were the author(s) able to answer the question (test the hypothesis) raised?
    4. Did the research provide new factual information, a new understanding of a phenomenon in the field, a new research technique?
    5. How was the significance of the work described?
    6. Did the reported observations/interpretations support or refute observations or interpretations made by other researchers?

Final Reflection Essay (Need this paper completed by 1pm tomorrow Eastern time)

To close the semester, you will write a 2-page reflection essay in which you examine your experiences and development since the first week of class. This essay prompts you to re-read your previous work and discuss your growth as a writer and reader. You will examine the most memorable highs and lows of the semester—your struggles, your successes, as well as those moments of which you are most proud. 

Preparing for the Essay

To prepare, re-read your previous work and assess your progress over the course of the semester. Reflect back over the semester, and think about the moments that stick out to you. Re-read your mid-term reflection essay and think about your progress since then. Open the syllabus and review the Learning Outcomes listed on the first page. You can find a link to the Syllabus in the Files tab. Compare your progress in these outcomes. 

Writing the Essay

The following generative questions are a good place to start. 

  • What were the most memorable moments in this class? Which assignments were the most challenging? Which were the most rewarding or engaging? Which reading or writing assignments do you think will “stick with” you?
  • How has this this class helped you to achieve the Learning Outcomes listed on the Syllabus? In which areas have you progressed the most? Which areas still need some work?

Drafting Your Essay

After doing some considerable brainstorming and reflective thinking (using the above prompts), you can begin drafting. There is no prescribed structure for this essay. However, you do need to think rhetorically about about your stylistic and organizational choices. This is a two-page essay, so you will need to compose several paragraphs. How those paragraphs are organized and developed is your rhetorical choice. Some options include, but are not limited to, the following organizational strategies:

  • Using an academic structure, with an introduction, body paragraphs, and a conclusion.
  • Using a narrative structure that builds chronologically from beginning to end. Tell the story of the semester.
  • Composing a set of vignettes–3 or more brief episodes or themes that work independently as well as together.  These vignettes could explain or narrate the 3 (or 4 or 5) most memorable/challenging/successful/etc. moments of the semester.

Your assignment needs to be submitted on time. Because of the tight turnaround for submitting final grades to the Registrar, I will not be able to accept late submissions.

Analyze the health status of a specific minority group. Select a minority group that is represented in the United States (examples include American Indian/Alaskan Native, Asian American, Black or African American, Hispanic or Latino, Native Hawaiian, or Pacific Islander).

In an essay of 750-1,000 words, compare and contrast the health status of the minority group you have selected to the national average. Consider the cultural, socioeconomic, and sociopolitical barriers to health. How do race, ethnicity, socioeconomic status, and education influence health for the minority group you have selected? Address the following in your essay:

Chapters 2 and 3 break down the federal courts and the state court.  Rodney King was an individual beaten by police officers in Los Angeles in the early 1990s.  This event led to riots and two court cases, one in the California court system and the other in the federal court system.  Do your own independent research on the Rodney King case.  The documentary below gives some insight into this tumultuous period in our nation’s history.  It is not unlike what we have been experiencing over the past several months:

Assignment 1: Communicating Effectively with Children

 

As you have explored this week, affirming communication focuses on respect for children and involves both speaking and listening. In this Application Assignment, you will more closely examine ways in which to communicate effectively with children in order to foster nurturing relationships, cooperation, and feelings of worth.

In preparation for this assignment:

Reflect on the articles in this week’s Required Resources, “The Nature of Teacher Talk during Small Group Activities” and “Stepping Back to Listen to Jeff: Conversations with a 2-Year-Old.” In each, the authors offer guidelines for facilitating affirming communication with children. Re-read the articles and identify three important principles you gleaned to guide you in fostering positive communication between you and the children with whom you work or may work with in the future.

Now, think about the media segment demonstration which features Lisa Kolbeck engaging two very quiet young children in conversation as they play. How well does Ms. Kolbeck exemplify these principles in her communication and interaction with the children in the media segment? It may be helpful to replay the media segment with these principles in mind and take note of specific comments that she makes or behavior that she engages in while she converses with the children. What additional kinds of interactions, communications, and sensitivities does she exhibit that help to draw children out and show respect for them as individuals?

Next, consider the article “Communicating with Babies” which discusses ways in which to appropriately communicate with infants. As you know, children of different ages communicate in different ways. Caring adults communicate in ways that relate to the age and interests of the child. Do the three guiding principles you identified earlier in this assignment hold up when working with infants as well? How so? If not, why not?

Finally, think about your own experiences in talking with and listening to young children. What additional insights might you have for facilitating affirming communication from your professional and/or personal perspective? What methods have you used to enhance positive communication with children to help them develop confidence, feelings of self-worth, and positive, respectful relationships with others? How do you know that these methods have been effective?

Foundational Neuroscience

The agonist-to-antagonist spectrum of psychopharmacologic agents

In Psychiatry, medications are generally small molecules and act differently (Berg & Clarke, 2018).  Traditional receptor theory states that ligands activate receptor sites and act as agonists with various degrees of intrinsic efficacy or as antagonists with zero intrinsic efficacy (Berg & Clarke, 2018).  Inverse agonists have the opposite effect of an agonist and reduce the “constitutive” activity of the receptor (Berg & Clarke, 2018).  Psychopharmacologic agents can be agonists, antagonists, and simultaneously agonists, antagonists, and inverse agonists acting at the same receptor (Berg & Clarke, 2018). Agonists act to mimic the action of an endogenous neurotransmitter (Berg & Clarke, 2018).  Antagonists block the effects of endogenous neurotransmitters and oppose normal synaptic transmission (Berg & Clarke, 2018).  Partial agonists act somewhat like agonists in that they directly act on receptors, but if used in the presence of an agonist, they compete for the receptor and have partial blocking properties; hence they are sometimes called agonist–antagonists (Berg & Clarke, 2018).

G Couple Proteins and Ion Gated Channels

A neurotransmitter can affect the activity of a postsynaptic cell via two different types of receptor proteins (Purves et al., 2001).  Ionotropic receptors are linked directly to ion gated channels. These receptors contain two features: an extracellular site that binds neurotransmitters and a “membrane-spanning domain” that forms an ion channel (Purves et al., 2001).  The second family of neurotransmitters receptors does not have ion channels as part of their structure; instead, they affect channels by activating intermediate molecules called G-proteins (Purves et al., 2001).  G protein-coupled receptors (GPCRs) are the largest known class of membrane receptors and are the target of about 30-50% of modern medicinal drugs (Purves et al., 2001).  When signaling molecules, or ligands, bind to GPCRs, G-protein activation triggers the production of second messengers, like hormones (Purves et al., 2001).  Like in GPCRs, ligands also bind to ion gated channels and initiate a chemical response.  Once the ligand binds to the allosteric site of the ligand-gated ion receptor, the channel opens, and the ion permeability of the entire plasma membrane can quickly change (Purves et al., 2001).  When the channel opens, ions like potassium, sodium, or calcium can move through the open channel, and an electrical signal is generated inside the cell (Purves et al., 2001).  Ligand-gated ion channel receptors generally mediate rapid postsynaptic effects, while activating metabotropic receptors (GPCRs) typically produce a much slower response (Purves et al., 2001).

Epigenetics

Epigenetics are chemical modifications that can silence or activate genes without modifying the nucleotide sequence (Stefanska & MacEwan, 2015).  It describes “genetic information that is ‘beyond’ or ‘above’ that information coded solely by our genetic code”  (Stefanska & MacEwan, 2015, p. 2702).  Often epigenetic variations are the cause of an underlying disease (Stefanska & MacEwan, 2015).  Drugs may not be designed to be as exact to a particular ligand or specific to a particular gene or protein subtype; they may indeed have to be able to be broader ‐ acting over a range of epigenetic large-scale events (Stefanska & MacEwan, 2015).  Pharmacological intervention may need to focus on one type of ligand or a particular gene. Still, rather drugs may need to be more “broad” to work more effectively against certain diseases (Stefanska & MacEwan, 2015).   Such knowledge can provide a strong biological foundation for developing better targeted personalized medication strategies (Stefanska & MacEwan, 2015). Epigenetic modification can be influenced by environmental factors such as recreational drugs, diet, and exercise (Stefanska & MacEwan, 2015).  “Transcription and numerous other genomic functions are epigenetically controlled via heritable but potentially reversible changes in DNA modification and histones (acetylation, methylation, phosphorylation)” (Browne et al., 2020, p. 22).  

Impact on patients

Psychiatric nurse practitioners need to consider epigenetics when prescribing medications.  An example would be in the treatment of patients who have opioid use disorder.  Susceptibility to opioid addiction is known to be strongly influenced by environmental factors. Thus, epigenetics could be important for understanding individual vulnerability to addiction and response to treatment (Hurd & O’Brien, 2018).  “The epigenetic mechanisms that turn genes on and off to set the state of gene expression patterns and thus cellular function include methylation of DNA and modifications (e.g., methylation, acetylation, and phosphorylation) of histones” (Hurd & O’Brien, 2018, p. 938).  An example of an epigenetic change in chronic heroin users includes increased methylation of the OPRM1 gene, which leads to reduced mu-opioid receptors(Hurd & O’Brien, 2018).  A reduction of mu-opioid receptors translates to a higher dose of opioids needed to satisfy the prior therapeutic effect (Hurd & O’Brien, 2018).  Frontline treatment of opioid addiction with mu OR agonists or partial agonists, such as methadone or buprenorphine, produces epigenetic modifications (Browne et al., 2020).
