Collaborative measures have proven to be more effective when there is a shared understanding of the focus, goals, and vision. Fullan and Quinn (2016) found that coherence emerges as members of the organization recognize what needs to be done and acknowledge their part in the process, and powerful things happen. This is critical within the field of special education. As a leader in the field of special education, collaboration is vital to the successful design and implementation of individualized instruction for students with exceptionalities. In addition, special education leaders must be savvy in collecting and analyzing data for program effectiveness and student achievement. Throughout this course, you will complete a five-part Course Project in which you collaborate with a team from a special education program to determine the program’s effectiveness, including strengths and needs of the program. Based on the identified strengths and needs, you will begin to create a plan for action to implement with the program team in the future. Please note that you are only creating the plan. You are not required to implement the plan.

For this Assignment, you will complete Part 1 of your Course Project, identifying a major challenge facing your organization at the school or district level. Based on the challenge identified, you will select a system-wide program or model in which you will continue to develop and implement throughout Modules 1–5.

  • An analysis of the special education program that will be the focus of this project. Include the type of program, a general description of the students with exceptionalities that are served within this program, how services are provided, and the professionals who work within the program.
  • Identify a group of special education stakeholders who you will invite to a meeting to evaluate the effectiveness of the program. Explain why each of the special education stakeholders invited is a key member of this team.

I take this discussion from when I first started at AIG.  I was a fresh graduate from my MBA/MIM schools and I was recruited to AIG’s Management Associate program in their life division.

Previous to getting my graduate degrees I had worked in non-profit with refugees, and the idea of working for ‘evil corporate America’ was somewhat in the back of my mind.  I always thought of myself in the rebel alliance fighting against the Empire.  However with a very first time pregnant wife I was just glad to have a job (this was right after 9/11 and there weren’t many companies hiring).

Wouldn’t you know it, my first job was to assess the situation in Latin America for a project that was somewhat outside of local legal regulations.  The Argentine Peso had just devalued to 30% of its original value and the fear in the region of currency devaluation was strong.  Therefore AIG wanted to set up an Offshore US Dollar based company that could sell high net worth dollar based policies into countries South of the boarder to HNW individuals.  

I helped to set up a company that was going to try to get investments into our insurance products that were really just a wrapper.  The product was mainly an investment product that was based in US dollars.  Our customers were going to have to give us US dollars, but then their investments would not be at risk of devaluing in their local currencies.  Technically this is not truly legal to sell in the countries and so we had to do so clandestinely.  The customer was supposed to only buy this outside of their own country, but we suspected the brokers who were selling this did not hold too hard to that rule.  It was a hard thing for me to get my head around.  

However, after seeing how much wealth had been wiped out in Argentina, I realized that had some of this wealth been in US dollars, the people would have been able to bring those dollars back ‘on shore’ and back into a faltering economy.

You are tasked with three things (keeping in mind this is 2001):

1)  Would you be OK doing this as your first job out of Grad school?  Why or Why not?

2)  Where do you base your company jurisdiction (this is where your insurance guarantees, or policy documents are located) from?  There are a few countries that are OK with hosting off-shore companies.  Find them and tell me which one you would chose and why.  Keep in mind you physically don’t have to be there, you just have to register your company there.  There are a couple of countries that specialize in off shoring.  Look them up.

3) You need to set up a dollar friendly operation center somewhere in Latin America.  Whatever country you chose to set this company up you may NOT sell to its citizens.  Which country do you chose to set up in and why?

4) Chose 5 countries you would enter to try to find HNW brokers to sell your product in South and Central America and tell me why you chose those 5 countries.

What Is Cognitive Behavioral Therapy?

Cognitive behavioral therapy (CBT) is a form of psychological treatment that has been demonstrated to be effective for a range of problems including depression, anxiety disorders, alcohol and drug use problems, marital problems, eating disorders and severe mental illness. Numerous research studies suggest that CBT leads to significant improvement in functioning and quality of life. In many studies, CBT has been demonstrated to be as effective as, or more effective than, other forms of psychological therapy or psychiatric medications.

It is important to emphasize that advances in CBT have been made on the basis of both research and clinical practice. Indeed, CBT is an approach for which there is ample scientific evidence that the methods that have been developed actually produce change. In this manner, CBT differs from many other forms of psychological treatment.

CBT is based on several core principles, including:

1.   Psychological problems are based, in part, on faulty or unhelpful ways of thinking.

2.   Psychological problems are based, in part, on learned patterns of unhelpful behavior.

3.   People suffering from psychological problems can learn better ways of coping with them, thereby relieving their symptoms and becoming more effective in their lives.

CBT treatment usually involves efforts to change thinking patterns. These strategies might include:

o Learning to recognize one’s distortions in thinking that are creating problems, and then to reevaluate them in light of reality.

o Gaining a better understanding of the behavior and motivation of others.

o Using problem-solving skills to cope with difficult situations.

             Many of the most popular and effective cognitive behavioral therapy techniques are applied to what psychologist call cognitive distortion, inaccurate thoughts that reinforce negative thought patterns or emotions. (Grohol,2016)

There are 15 main cognitive distortions that can plague even most balanced thinkers.

1.      Filtering   refers to the way a person can ignore all the positive and good things in life to focus solely on the negative. It’s the trap of dwelling on a single negative aspect of a situation, even when surrounded by an abundance of good thinks.

2.      Polarize thinking/Black-and-white thinking   This cognitive distortion is all-or-nothing, never shades of gray. If you don’t perform perfectly in some area, then your may see yourself as a total failure instead of simply recognizing that you may be unskilled in one area.

3.      Overgeneralization    is thinking a single incident or point in time and using it as the sole piece of evidence for a broad conclusion.

4.      Jumping to conclusions.   Like overgeneralization, this distortion involves faulty reasoning in how one makes conclusions. Unlike overgeneralizing one incident, jumping to conclusions refers to the tendency to be sure of something without any evidence at all.

For example, we might believe that our fears will come true before we have a chance to really find out.

5.      Catastrophizing/Magnifying or Minimizing   This distortion involves expecting that the worst will happen or has happened, based on an incident that is nowhere near as catastrophic as it is made out to be. For example, you may make a small mistake at work and be convinced that it will ruin the project you are working on, that your boss will be furious, and that you will lose your job.

Alternatively, one might minimize the importance of positive things, such as an accomplishment at work or a desirable personal characteristic.

6.      Personalization   This is a distortion where an individual believes that everything, they do have an impact on external events or other people, no matter how irrational that may be. A person with this distortion will feel that he or she has an exaggerated role in the bad things that happen around them.

Example, for instance, a person may believe that arriving a few minutes late to a meeting led to it being derailed and that everything would have been fine it they where on time.

7.      Control fallacies   This distortion involves feeling like everything that happens to you are either a result of purely external forces or entirely due to your own actions. Sometimes what happens to us is due to forces we cannot control, and sometimes what it’s due to our own actions, but the distortion is assuming that it is always one or the other.

We might assume that difficult coworkers are to blame for our less-than-stellar work, or alternatively assume that every mistake another person makes is because of something we did.

8.      Fallacy of fairness   We are often concerned about fairness, but this concern can be taken to extremes. As we all know, life is not always fair. The person who goes through life looking for fairness in all their experiences will end up resentful and unhappy.

Sometimes things will go our way, and sometimes they will not, regardless of how fair it may seem.

9.      Blaming   When things do not go our way, there are many ways we can explain assign           responsibility for the outcome. One method of assigning responsibility is blaming others for what goes wrong.

Sometimes we may blame others for making us feel or act a certain way, but this is a cognitive distortion. Only you are responsible for the way you feel or act.

10.  “Shoulds”   Refer to the implicit or explicit rules we have about we and others should behave. When others break our rules, we are upset. When we break our own rules, we feel guilty.

For example, we may have an unofficial rule that customer service representatives should always be accommodating to the customer.

When we interact with a customer service representative that is not immediately accommodating, we might get angry. If we have an implicit rule that we are irresponsible if we spend money on unnecessary things, we may feel exceedingly guilty when we spend even a small amount of money on something we do not need.

11.  Emotional reasoning   This distortion involves thinking that if we feel a certain way, it must be truth. For example, if we feel unattractive or uninteresting in the current moment, we think we are unattractive or uninteresting. This cognitive distortion boils down to:

“I feel it, therefore it must be true”

Clearly, our emotions are not always indicative of objective truth, but it can be difficult to look past how we feel.

12.  Fallacy of change   The fallacy of change lies in expecting other people to change as is suits us. This ties into the feeling that our happiness depends on other people, and their unwillingness or inability to change, even if we demand it, keeps us from being happy.

This is damaging way to think because no one is responsible for our own happiness except ourselves.

13.  Global labeling/mislabeling   This cognitive distortion is an extreme form of generalizing, in wish we generalize one or two instances or qualities into a global judgement. For example, if we fail at a specific task, we may conclude that we are a total failure in not only that area but all areas.

Alternatively, when a stranger says something a bit rude, we may conclude that he or she is an unfriendly person in general. Mislabeling is specific to using exaggerated and emotionally loaded language, such as saying a woman has abandoned her children when she leaves her children with the babysitter to enjoy a night out.  

14.  Always being right   While we all enjoy being right, this distortion makes us think we must be right, that being wrong is unacceptable. We may believe that being right is more important than the feelings of others, being able to admit when we have made a mistake or being fair and objective.

15.  Heaven’s reward fallacy   This distortion involves expecting that any sacrifice or self-denial will pay off. We may consider this Karma, and expect that Karma will always immediately reward us for our good deeds. This result in feeling of bitterness when we do not receive our rewards (Grohol, 2016).

Many tools and techniques found in cognitive behavioral therapy are intended to address or reverse this cognitive distortion.

Discussion

As primary care providers, it is our responsibility to understand the efficacy, safety, appropriateness of contraceptive methods for our patients.  Given Karen’s history of menorrhagia and dysmenorrhea, migraines, tobacco use, and family history, elevated blood pressure, and lack of desire to become pregnant, I would recommend a contraceptive implant/intrauterine device (IUDs) or a tubal ligation/sterilization.  The increased usage of intrauterine devices has contributed to decreased pregnancy rates in the United States.  Evidence based practices have amplified the figure of women who are medically entitled for intrauterine devices and have permitted expedited access to the methods (Nelson & Massoudi, 2016).  Presently, women can choose from three hormonal IUDs, the Mirena, Liletta, and the Skyla; or they can choose the only non-hormonal IUD, the Paragard (Nelson & Massoudi, 2016).  In Karen’s case, I would advise that she utilize one of the hormonal IUD options.  Additionally, if Karen chooses not to utilize a hormonal IUD, the next option she would be medically eligible for is a tubal ligation/sterilization.  Nationally, more than 220 million couples chose sterilization as a desired form of contraception, as it has been proven to be safe and effective for those no longer desiring to reproduce (Sung & Abramovitz, 2021).

Identify one method that you feel would be most beneficial to Karen and discuss why you selected it.

A conversation should take place with Karen to determine whether or not she desires additional children in the future.  Her decision would ultimately impact the method of contraception I would recommend to benefit her well-being and family planning goals.  Given the fact that Karen is in a new relationship, her relationship may escalate with her current partner.  Her partner may communicate his desire to have children, which may alter her views on reproduction.  With that being said, I would recommend that Karen utilize the Mirena.  Studies have found the Mirena to be the most effective medical therapy for menorrhagia and dysmenorrhea, and has improved the lives of approximately 94.6% of women after using the Mirena for 6-12 months (Nelson & Massoudi, 2016). This method will offer her a period benefit by decreasing blood loss, prevent unwanted pregnancy, is reversible, and possess a decreased risk for cardiovascular disease and venous thromboembolism given her history. 

Are there any methods that you would not recommend for Karen? Why?

The first method of contraception that I would not recommend for Karen, is the Paragard Intrauterine device considering she is currently suffering from menorrhagia and dysmenorrhea.  According to Nelson and Massoudi (2016), studies have found dysmenorrhea and abnormal bleeding patterns are associated with the Paragard, causing an increase of blood loss by 30%-50%.  The second method of contraceptive I would not recommend for use are combined hormonal contraceptives, such as the pill, transdermal patch, and ring.  Studies have shown there is an increased risk for venous thromboembolisms and cardiovascular disease associated with certain disease processes and social habits.  Due to these risks, contraindications are in place for the utilization of combined hormonal contraceptives for women with the following issues: a history of migraines, uncontrolled hypertension, age of 35 years or older with a smoking history, ischemic heart disease, past or active history of valvular heart disease, endometrial cancer or breast cancer (Cooper & Mahdy, 2021).  Thirdly, I would not recommend the use of the Depo-Provera shot, as studies have found reduced bone mineral density with prolonged use and an increase in weight, approximately 2-3 kg per year (Khadilkar, 2018).
